Project Background
PROJECT LOCATION – TRANSPORTATION TO ISLAND IS BIDDERS RESPONSABILITY BY PRIVATE/CHARTERED BOAT/AIRCRAFT NO DAILY FERRY SERVICE AVAILABLE.
VISITATION TO THE SITE The areas to be drywalled are attached. The approximate North Bass Lake House area has 4 bedrooms, kitchen, dining room, recreation room, living room, mini kitchen area, foyer, 4 hallways, and laundry area which adds up to APPROXIMATELY 2,600 SF. The contractor shall field measure carefully and check all areas for other conditions to ensure proper fit in all areas designated. When visiting the site contractors shall make an appointment with the Maintenance Director, Ross Muszynski, 419-341-4459 between the hours of 8:30am to 3:00pm.

Specifications/Scope of Work
All material and labor to be supplied by Contractor:
• Hang, mud/tape/sand, and texture each unit. 5/8” sheetrock on the ceilings, 1/2” sheetrock on the walls, unless otherwise noted on plans. Note: 5/8” sheetrock required on the interior of the unit party wall.

• Floors protected before mudding/taping begins. Floors cleaned during final clean-up of drywall phase. Trash to be hauled away by Contractor, NO DUMPSTER is available.
• Excess drywall/material cleaned up and removed from site by contractor no trash facility on site.
• All equipment and fuel to heat/dry mud, unless otherwise agreed upon with Owner. Propane/kerosene heaters are not permitted on the jobsite.
CLEAN UP After completion of installation, the installation contractor shall remove all scraps, packaging, and rubbish caused by his work.

Site Visit & Point of Contact
Prior to submitting a bid response, offerors are encouraged to visit the park. Contact the Maintenance Director, Ross Muszynski, 419-341-4459 between the hours of 8:30am to 3:00pm.
Once a contract is awarded, failure of the bidder to have requested a site visit to become familiar with the facility and requirements of the bid will be insufficient reason to support any request to be released from the contract.
Project Award
For bids involving service projects on DNR property, the lowest apparent bidder will be required to provide 2 examples of similar completed projects including photos.  Provide the person’s name, organization name, phone number and email address for each project example.  In addition, the bidder must complete a pre-award meeting with appropriate DNR staff to ensure that the materials and scope are mutually understood.
Evaluation. The contract will be awarded to the lowest responsive and responsible bidder as determined by the State.
Contract Award. The contract will be awarded to the lowest responsive and responsible bidder by low lot total.  Low lot total will be determined by multiplying the unit cost by the estimated usage listed in the bid and then adding each of the totals together to arrive at a total for all items. Failure to bid all items may result in the bidder being deemed not responsive.
For the purchase of services:
The awarded supplier will be required to sign an Addendum to Bid Quote or Personal Services Agreement.  Example attached.

All invoices must be submitted DIRECTLY to Financial Shared Services (FSS).
There are three options available to submit invoices to FSS: In order to process your invoice, please make sure all the required information listed below is on your invoice before submitting it to FSS or they will be returned for not being a proper invoice.
  1. Must include the Purchase Order (PO) number assigned.  You may shorten it to the last five digits.
  2. Supplier full name as it appears on your PO.
  3. The Ship to and/or service facility name and full address
  4. Unique invoice number
  5. Date of service or date work was completed.
  6. Complete itemization of services performed, materials or goods supplied and/or labor furnished.
  7. Line charge(s).  If parts are included, please break out the line charges with product and charges separately.
  8. No State of Ohio sales tax
  9. Remit to address must match our records for payment
  10. This PO is only valid for the goods and/or services received through June 30, 2025.
  11. Any goods/services received after June 30, 2025, will require a new PO.


All fields mentioned above are required to count as a correct invoice; failure to provide this information will result in a returned invoice and delayed payment.
