District 14,
The purpose of this project is to establish sustainable, outpatient clinic-based non-oncological infusion therapy services through a partnership between Central Health and CommUnityCare. This initiative will expand timely access to medically necessary treatments, improve care coordination across primary and specialty services, and reduce avoidable emergency department utilization for eligible patients.
