Ohio Department of Health-DOH57300-Bureau of Health Improvement and Wellness-Cardiovascular Health Program-Cardiovascular Health (CVH) Community/Clinical Linkage Project- RDQ

The ODH Cardiovascular Health (CVH) Program is seeking a contractor to collaborate with Unite Us (a coordinated care network of health, human, and social services providers), to continue expansion of the geographic reach of healthcare systems/providers who participate in the Unite Us electronic referral network as well as increase the number of community-based social service organizations who join the network. This project will simplify referrals to community resources to address the social determinant of health (SDoH) needs for individuals at risk for cardiovascular disease (CVD). In addition, the contractor will provide support and technical assistance (TA) to providers participating in a CVH QIP on how to utilize the Unite US platform for referrals and bi-directional communication.

Offerors will need to review the attached RDQ DOH57300 SRC0000029042. Each Offeror must submit a technical proposal and cost proposal as part of its total Proposal before the opening time on the Proposal due date. Further, the Offeror must title the electronic files with “DOH RDQ #SRC0000029042/DOH57300 - Technical Proposal” and "DOH RDQ #SRC0000029042/DOH57300 - Cost Template” as appropriate. Please note there is not a page minimum or maxium for the technical proposal and bidders should refer to technical evaluation criterion on the RDQ document for guidance.
