Summary
  1. PURPOSE
The Ohio Department of Mental Health and Addiction Services ( Ohio MHAS) is seeking proposals from qualified contractors to provide on-site Yoga Instruction at Heartland Behavioral Healthcare, located at 3000 Erie St. South, Massillon, Ohio 44646 .
  1. BACKGROUND
Heartland Behavioral Healthcare is a 152-bed inpatient care facility for acute mentally ill adults.  The service contractor shall provide Yoga Instruction for our patients which can greatly enhance their healing process .
SCOPE OF WORK AND DELIVERABLES

The yoga Instructor is to conduct yoga classes with the patients at Heartland Behavioral Healthcare. Yoga will be conducted up to 10 hours weekly on the grounds of Heartland Behavioral Healthcare. Yoga Instructor must be able to work with a diverse population with mental health diagnoses and behavioral issues. Through yoga, the instructor will provide patients with benefits such as stress reduction, sense of personal control, improved flexibility and muscle strength, and improved balance. Yoga Instructor will offer 45 min - 1 hour group sessions with a maximum of 10 hours per week.

V. MINIMUM QUALIFICATIONS OF CONTRACTOR
Contractor shall be certified through a nationally recognized yoga school. Must possess 2 or more years of Yoga Experience. Experience required working with patients who have a mental illness diagnosis.
Contractor shall not be subject to an “unresolved” finding for recovery under Section 9.24 of Ohio Revised Code.

Process
EVALUATION OF PROPOSALS
  1. EVALUATION PROCESS

MHAS’s evaluation process of responses submitted to this request may consist of up to four distinct phases:
    1. MHAS’s initial review of all proposals for timely submission;
    2. An evaluation committee review of the proposals for defects and scoring;
    3. MHAS’s request for more information (clarifications, interviews, presentations, and/or demonstrations); and,
    4. Negotiations or best offer requests.

At its sole discretion, MHAS will determine whether phases three and/or four are necessary under this RFP, reserving for itself the ability to eliminate or add phases three or four at any time during the evaluation process. MHAS may add or remove sub-phases to phases 2 through 4 at any time if MHAS believes doing so will improve the evaluation process.
  1. PROPOSAL EVALUATION CRITERIA

In the proposal evaluation phase, MHAS staff or reviewers selected by MHAS (the committee) will rate the proposals submitted in response to this RFP based on the following criteria and weight assigned to each criterion.

Proposer has submitted a cost propsal that is complete, and the quality of the proposal demonstates porposer's ability to sucessfully complete all deliverables within a budget considered reasonable. 10
