I. Project Information
A. REQUEST FOR PROPOSAL (RFP) NOTICE
The City of Daly City (City) hereby requests proposals from qualified firms to provide construction
inspection/support services for the Westlake Slurry Seal Project in Daly City. The project bids will
be opened on June 2, 2026. It is anticipated that the project will be awarded in June 2026. Upon
request, the construction documents will be made available to inquiring firms. All questions
regarding this Request for Proposals (RFP) shall be directed to Benjamin Zee, Engineer I, at
bzee@dalycity.org. Questions submitted after the questions submittal deadline listed in this RFP
may not be answered. At the City’s discretion, a list of questions and answers may be published.
Proposals shall be submitted by firms that have a capable and demonstratable background in the
type of work described in Section C, "SCOPE OF SERVICES," of this notice. In addition, all
interested firms shall have sufficient, readily available resources in the form of trained personnel,
support services, specialized consultants, and financial resources to carry out the work without
delay or shortcomings.
The proposals shall be submitted electronically to the Engineering Division of the Public Works
Department, City of Daly City, by e-mail to bzee@dalycity.org no later than:
4:00 p.m. on June 23, 2026
Each proposer shall submit one (1) electronic copy of the proposal in PDF format in accordance
with Section II, "SUBMITTAL REQUIREMENTS."

B. PROJECT DESCRIPTION
The consultant shall provide construction inspection and support services required for the
complete construction of the project. The construction costs for the Westlake Slurry Seal are
approximately $3.0 million. The time of completion established for contract work is one hundred
twenty-five (125) working days. The project will include:
• Asphalt concrete (AC) deep-lift repairs
• Crack seal
• Protection of utility manhole frames and covers, utility boxes, and monument boxes
• Application of slurry seal
• Removal and application of pavement striping and markings
• Traffic control
• Removal and replacement of sanitary sewer and storm drain manhole frame and covers
to finished grade
• Removal and replacement of water valve frame and cover to finished grade
• Removal and replacement of monument box and cover to finished grade
• Adjustment of sanitary sewer and storm drain manhole frame and covers including utility
boxes, monument boxes and water valve box/lid to finished grade
• Street sweeping/cleaning
C. REQUESTED SCOPE OF SERVICES
The Consultant shall perform the following professional services described in this section.
PART I. INSPECTION
Project inspection shall be full-time by qualified personnel throughout the duration of the
project. Inspector will include construction observation and acceptance of testing as specified in
contract specifications. Digital still photos and video records shall be taken of work progress to
document the Contractor’s work. All construction activities shall be coded using the CPM
schedule activity numbers and bid item number.
Project inspector shall observe, monitor, inspect, and document all aspects of the contractor’s
daily activities including manpower, equipment, and materials. The project inspector shall ensure
the prosecution of the work is in accordance with the approved contract documents.
Due to the potential of concurrent activities taking place at various locations and dates,
consultant shall provide available qualified full-time inspectors to ensure all activities are under
supervisions at all times.
Project inspector(s) shall ensure the Contractor’s temporary installations including, but not
limited to traffic controls, material storage, and work areas are provided and maintained in
accordance with approved plans, City standards, industry practices and OSHA guidelines, and that
the ROW is safely accessible by both vehicles and pedestrians as intended.

II. Submittal Requirements
A. PROPOSAL REQUIREMENTS
The pre-qualified consultant shall submit one (1) electronic version (PDF format) of the proposal
containing 8-1/2” x 11” sheet sizes for the text and 11” x 17” sheet sizes for any large drawings.
The proposals should be no more than 15 pages. The Cover, Table of Contents, and resumes of
key personnel will not be counted towards the 15-page limit. Consultants shall submit electronic
proposals to:

The proposal must include, at a minimum, the following elements:
1. Letter of Transmittal
a. To the attention of: Mr. Benjamin Zee, Engineer I, Engineering Division, Public
Works Department, 333 - 90th Street, Daly City CA 94015.
b. The letter must name all persons authorized to make representations for the
Proposer, such as sub-consultants, and include the titles, addresses and telephone
numbers for such persons. An official authorized to negotiate for the Proposer
must sign the Letter of Transmittal.
2. Acknowledgement of Addenda
a. If any addenda are issued, you must acknowledge your receipt of them either by
including a statement in your transmittal letter or by returning signed addenda
with the proposal.
3. Firm Profile and Project Team
a. Prime(s): Provide a detailed resume of the proposed principal-in-charge, the
project manager (PM), and the lead engineer/inspector who will be responsible
for this project. The PM shall be a full-time employee of the prime(s). Clearly
identify each individual’s relevant experience (former projects) with photos.

b. Sub-Consultants: Provide a detailed resume of the proposed project manager
(PM), who shall be a full-time employee of each sub-consultant for this project.
Clearly identify the PM’s relevant experience (former projects) with photos.
c. Confirm that the key project personnel identified by the Prime Consultant and
Subconsultants shall not be substituted without approval of the City. The City must
also approve any new, key team members.
4. Relevant Experience – Prime Consultant and Sub-Consultants
a. Describe experience in providing the necessary services for at least three (3)
projects similar in size and scope to this project. For each project, provide the
following information:
• Client name, project name and location.
• Description of project scope.
• Month and year commenced and was (or will be) completed.
• Project construction cost.
• Contract amount.
• Firm’s project responsibility.
• Names of key personnel involved in working on the projects.
• Two (2) client references for each project, including contact names,
addresses, and telephone numbers.
b. Describe issues or problems during design or during the administration of the
project and how these issues were resolved.
c. Describe key take-aways or lessons learned from these projects that could have
helped mitigate some of the issues in sub-section b. above on this project or future
projects.
5. Project Approach, Organization, and Local Presence
a. With reference to Section I “Project Information” of this RFP, present your
approach and organization for providing services on City of Daly City project.
Indicate your understanding of the critical project elements, and what special
approaches your team will feature to manage these elements.
b. Describe how you intend to interface with City staff.
B. COST PROPOSAL AND HOURLY BILLING RATES
A cost proposal and hourly billing rates shall be submitted in a separate PDF file, and shall include
at minimum the following information:
1. Consultant fee schedule for the duration of the project, including proposed level of effort
(in hours and cost) for each major task, proposed level of effort for optional tasks as
