Occupational Health Consultant- New Employee Physicals
(Office of Nursing and Allied Health Shelby County Health Department)
TO ALL PROSPECTIVE BIDDERS:
The following questions were submitted by a potential bidder. Our answers are listed
below in red:
1. How many exams are needed annually?
The number of physical exams (PEs) vary per month.
The average number will vary from 180-300. 180 occurred, due to no
family nurse practitioner being available for 6 months in 2025.
2. Can you provide a list of components that are currently being used in your
exams?
Head to toe physical exam
Vision
Audiometric (hearing)
Chest (x-ray) PA & lateral) with interpretation
Lab- CBC, LPP, CMP (fasting)
Complete urinalysis
EKG as indicated by medical history or physical
QFT- TB lab test. Follow-up with the TB clinic if positive
OSHA Respirator evaluation (fire fighter only)
Pulmonary Function Test (spirometry-breathing)
Medical confirmation (required if carrying a weapon)
3. For these exams is drug and/or alcohol testing needed?
Yes
4. What departments will utilize these services?
Shelby County Fire Department
Sheriff’s Office
Corrections Department
Support Services
Code Enforcement
Roads & Bridges
Environmental Services
Health Services
5. Are there OSHA requirements mandating specific components for these exams?
Yes, Respirator Evaluation
6. What is your expectation of hours regarding weekend and holidays?
The expected hours are Monday through Friday 8:00-5:00 pm.
7. Are there any requirements for the vendor to provide services onsite at Shelby
County locations?
There are no requirements for the vendor to provide services onsite at
SCG locations.
8. If needed, are we able to utilize sub-contractors for services such as ultrasounds,
stress testing, and advanced imaging?
Subcontractors will be utilized for EKGs, interpretation of Chest x-rays,
mental health evaluation, and drug testing.
