Northcoast Behavioral Healthcare -Intensive Care Room Doors

Contractor to Provide 2 Attic Stock and Install Eight (8) New Metal Steelcraft prepped 16 Gauge Doors for 4-0 x 7-0 openings, doors to have new vision lite kits with polycarbonate material to match existing thickness and size using existing full mortise continuous hinge or replace as needed. Door prepared for existing Best mortise lockset, keyed both sides and door to include top caps.  All screws to be tamper resistant. Change existing door systems with like materials including all paint, hardware, and wall repair.  A site visit is required to verify all sizes, handling, and prep.  An appointment can be scheduled by contacting Dave Palmer at (330) 467-7131 ext. 2243 or David.Palmer@mha.ohio.gov

1. OhioMHAS’s initial review of all proposals for timely submission;
2. An evaluation committee review of the proposals for defects and scoring;
3. OhioMHAS’s request for more information (clarifications, interviews, presentations, and/or demonstrations); and,
4. Negotiations or best offer requests.

At its sole discretion, OhioMHAS will determine whether phases three and/or four are necessary under this RFP, reserving for itself the ability to eliminate or add phases three or four at any time during the evaluation process. OhioMHAS may add or remove sub-phases to phases 2 through 4 at any time if MHAS believes doing so will improve the evaluation process.
