ADDENDUM NO. 1
The purpose of this addendum is to extend the current solicitation due date and time. All Responders may acknowledge receipt of this addendum by responding to Event No. 17237, Version 2 (or most recent version) by the solicitation due date and time. The due date has been extended to 3 P.M. on June 20, 2025, to permit responders additional time to submit responses.

THIS IS A FORMAL REQUEST FOR PROPOSAL (RFP). Non-electronic and e-mail responses WILL NOT BE ACCEPTED. The response must be submitted in the Supplier Portal no later than the date and time called for in the Finish Time in the Event Details.
PURPOSE AND BACKGROUND. The purpose of the Solicitation is to contract with a responder(s) to provide Food Service for Minnesota Direct Care and Treatment Bemidji Community Behavioral Health Hospital, contract release number F-515.
SUBMITTING QUESTIONS. Questions from prospective responders regarding this solicitation Must be submitted in writing to Rhiannon.Fissehaye@state.mn.us no later than June 11, 2025, at 3:00 PM Central Time. All questions received by the cutoff date and time will be responded to via an addendum. Contact regarding this solicitation with any State personnel other than the Buyer listed above may result in rejection of the response.

• Attachment A: Responder Declarations
• Attachment B: Exceptions to State's Terms and Conditions
• Attachment D: Responder Forms
o Workforce Certification
o Equal Pay Certificate Form

o Vendor License
o Certified Food Protection Manager Certificate (CFPM)
o Credential Verification of the Registered Dietician by the Commission of Dietetic Registration
