Summary
FY26/27 • The kitchen ANSUL hood suppression system needs inspected at 6-month intervals. Inspection should cover testing, servicing and any hydrostatic pressure testing required of the suppression system. This request should include repair of any parts for the system to keep it in service. This system was installed in 2016. The next hydro is due in 2028. The hood suppression system shall also be verified that it still sends an appropriate signal to the fire alarm panel. The testing of the system must occur after the kitchen closes for the day at 1:00p. Typical intervals for this testing is December and June. • Currently, DSFM has approximately 90 total, of various types, fire extinguishers located throughout the property and buildings. These are in-service extinguishers. Any extinguisher that would need to take off property for maintenance must have a “loaner” provided in its place. The agreement shall cover any inspection, testing, servicing and hydro inspection that would be required and the repair of any parts for the extinguishers to keep them in service. Typical interval for this annual service is in December. With exception to any extinguishers that are used between now and December, there are approximately ten (10) units that are due for maintenance in 2025, another ten (10) due in 2026, and approximately twelve (12) due in 2027. DSFM prefers to have the report of inspection at time of completion for Fire Code Compliance. Any work that does not fall within the scope must be given in the form of a quote and no work can be started or completed without a written notice to proceed. DSFM admin personnel will coordinate the scheduling. DSFM will assist with access and site expertise when needed. All work must be done, in accordance with Ohio law and manufacturer recommendations. All work will be performed between 8a-4:30p, Monday through Friday, excluding holidays.
Process

Itemized Quote. The quote shall be itemized and include the following: estimated shipping, company name, contact information, including name, phone number and email to which COM should remit the purchase order.
Nothing herein guarantees that the quote will be accepted by Commerce.
Form of Payment. Please state in the quote if you accept credit card as form of payment and if there are any additional fees applied to those purchases.  If so, please state the additional fees and/or surcharge in the quote.
Current State Contract or Agreement. If the products and/or services are currently contracted under a Department of Administrative Services (“DAS”) Contract, State Term Schedule (“STS”), Master Maintenance Agreement (“MMA”) or Master Cloud Services Agreement (“MCSA”), provide the contract number and confirm your quote pricing matches what is listed in the pricing section of the DAS contract.  If the pricing contained in your responsive documents fails to match the DAS contract pricing, your quote may be rejected.
If the product and/or service requested is not quoted from an existing DAS Contract or Agreement, please provide supplier’s Software License Agreement, Maintenance Agreement, and Terms and Conditions documentation when quoting.
If not all products and/or services are referenced on a DAS Contract, please provide two separate quotes: one for products and/or services addressed in the DAS contract and one for the products and/or services not addressed by the DAS contract.
If signature is required for products and/or services not addressed by the DAS contract, please provide the agreement and/or terms and conditions related to the purchase. This includes software license agreements.
