Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service-Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) Unique Entity ID (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below.

The VA Loma Linda (VALL) located at 11201 Benton St., Loma Linda, CA 92357 is seeking a potential qualified contractor that shall furnish training supervision, labor, transportation equipment, materials, supplies, and tools, to provide on-site training for the facilities irrigation system.

Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service-Disabled Veteran Owned Small Business set aside. However, if response by Service-Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 811210 Electronic and Precision Equipment Repair and Maintenance ($34 Million Dollars).
Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov. Additionally, all Service-Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the U.S. Small Business Administration located at https://veterans.certify.sba.gov All interested Offerors should submit information by e-mail to william.cornick@va.gov. All information submissions to be marked Attn: William Cornick, Contract Specialist and should be received no later than 4:30 pm PST on October 24, 2024.
1. Scope
The service contract shall provide preventive maintenance and repair service on Carestream Digital Radiographic system at LOMA LINDA VA MEDICAL CENTER.
The contractor shall perform corrective maintenance on the system if it shall be malfunctioning or inoperable at a mutually agreeable time and loaner equipment, when deemed necessary by the Contractor.
Training materials shall be provided for base software upgrades that affect user operation of the base software.
Must use OEM certified parts and service engineer/technicians must be OEM trained on all equipment included in agreement.
All repairs must follow OEM repair procedures and validation process to return the covered system to good working condition.
Perform planned maintenance service as required and must be in compliance with the OEM requirements.
24 hours, 7 days/week toll free support.
Unlimited service calls with on-site response within 4 hours (Monday to Friday 8:00 AM to 5:00 PM)
All parts covered on all systems listed in the equipment section of this SOW
All shipping for parts, labor, and travel costs will be included in the cost of the contract.
Support to include software updates and patches.
Equipment is not approved for remote access until proper approvals have been processed (MOU, ISA, ERA). All work must be completed on-site.
Response: All requests for repair services will be considered requests for emergency service unless a statement to the contrary is made by the COR, POC, or designee. A competent service representative adequately equipped with tools, test instruments, service manuals, and repair parts will arrive at VALLHCS within the designated response time. Such contacts, however, will not serve to alter the requirements for arrival on-site except when the request is withdrawn by Bio-Medical Engineering.
2. Qualifications
The contractor must meet The Joint Commission requirements for proving competency of service provider. To be considered eligible for consideration, offerors must have a field service representative that has been trained by the Original Equipment Manufacturer (OEM) on the specific model of equipment he/she shall be asked to work on. Contractor must provide, upon request, documentation of factory certified service/maintenance training on the specific equipment under the terms of this Purchase Order.

4. Performance
Provide technical phone support 24 hrs/day, 7 days/week.
Respond to customer by phone within 60 minutes.
Maintenance and repairs will be scheduled with a contact person as assigned by the COR, POC, or designee. The Bio-Medical Engineering Manager can be contacted at (909) 825-7084, x1716.
Normal duty hours are 8:00 a.m. through 5:00 p.m. PST, Monday through Friday, not including federal holidays.
Contractor shall provide the necessary manpower and supervision to properly execute the preventive and corrective maintenance of all systems covered in this contract.
A Field Service Report (FSR) must be generated and a copy given to the LOMA LINDA VA MEDICAL CENTER s Contract Officer s Representative (COR) or designee after completion of the preventive maintenance and corrective maintenance no later than five (5) days after service. It shall be preferred that this FSR be provided electronically if possible.
Preventive Maintenance shall be performed onsite by the contractor at the intervals that shall be in accordance with the manufacturer s recommendations, but no less than annually. Preventative maintenance shall be performed to manufacturer s specifications. Contractor shall furnish any required parts, supplies, or PM kits needed to perform Preventive Maintenance.
Perform system updates, refresh and applicable releases as required by the OEM.
Corrective Maintenance shall be performed onsite by the contractor. Contractor shall use commercially reasonable efforts to provide service as quickly as possible and on a priority basis for emergencies. Contractor shall ensure prompt communication with COR or designee. Corrective Maintenance shall not be an additional cost.
Contractor shall arrive at the designated location within four business hours of receiving the initial service request.
Contractor shall use genuine OEM replacement parts for all repairs and maintenance covered in the service plan.
Contractor shall provide on-site preventive maintenance as defined by the OEM equipment maintenance procedures in the service manual on equipment covered under a full service coverage plan or a preventive maintenance plan at no additional charge.
For equipment on plans that specify call continuation and to avoid disrupting clinical operations, contractor shall schedule on-site preventive maintenance outside of normal business hours when requested, provided the customer permits the service to start no less than 30 minutes before the end of the coverage period or no more than one hour before the start of the coverage period.
Repairs or services performed during normal working hours, the Contractor shall check-in with Police Services and the COR before proceeding to the job site. Contractor shall check-out with the COR and Police Services before leaving the job site. Note: Both check-in and check-out procedures are mandatory and is strictly enforced.
Failure to carry out the above procedures will be considered as a failure to document the service visit and will constitute a failure to perform.
At the completion of the service call, the service representative will submit a complete service report to Bio-Medical Engineering, as described in section 5, Documentation.
5. Acceptance
All completed work will be inspected and tested by a Radiographic Technician and the Contracting
Officer Representative (COR) for compliance and operational capabilities.
6. Documentation
All service reports submitted shall be legible and contain the following:
Date of service.
Complete description of equipment serviced including model and serial number.
Safety checks, performance data, and recommendations on maintaining the equipment in optimum operating condition.
Complete list of all materials, parts, or assemblies installed or replaced, including serial numbers of parts were applicable.
Labor hours.
Travel time.
Any unsafe condition or signs of misuse or neglect.
Purchase order number.
COR s signature after acceptance.
7. Removal of Equipment
Whenever possible, all repairs and preventative maintenance will be performed at VALLHCS. When that is not possible, The COR, POC, or designee may give approval for work to be done at another location. All transportation charges will be borne by the contractor.
VA property may not be removed from VALLHCS without a signed property pass. The property pass may be obtained after the COR authorizes the removal. Computer hard drives and other storage media items shall not leave VA property without VALLHCS s Information Security Officer s approval.
Requirements for service reports for designated equipment not at this location are the same as for work done on-site. Service reports for incoming equipment will be presented for signature at the time delivery. If equipment is returned via commercial carrier, the service report may be included with the shipment.
8. Response to Service Requests:
The Contractor must respond to requests for emergency service in the following manner:
A competent service representative adequately equipped with tools, test instruments, and repair parts will arrive at the VALLHCS within the designated response time.
The competent service representative will be prepared to begin work immediately to resolve the complaint as expressed by the COR, POC, or designee when service was requested. Work will continue without interruption until the problem is resolved.
The service representative will come equipped with all the service/technical manuals needed to perform the work.
The service representative may contact VA staff by phone before arrival at the VALLHCS. Such
contacts, however, will not serve to alter the requirements for arrival on-site except when the request for service is withdrawn by VALLHCS.
The designated response time for this contract is 4 hours from the time of the request for service.
If repeated requests are made before the arrival of contractor s repairer, the first request will determine the required time of arrival.
All requests for repair service will be considered requests for emergency service unless a
statement to the contrary is made by the COR, POC, or designee. Such statements will be
documented on the service report.
