Ohio Department of Health-DOH57310-Bureau of Maternal Child & Family Health-School Health Medical Consultant RDQ

The purpose of this contract is to provide medical consultation for all school - based services and programs offered by the ODH. In addition, the contractor will review TA (technical assistance) resources, make recommendations for program improvements and provide guidance on school health policy and procedures as needed by the Director of Health.

Offerors will need to review the attached RDQ DOH57310-BMCFH-School Health Medical Consultant #SRC0000031104. Each Offeror must submit a technical proposal and cost form/proposal as part of its total Proposal before the closing date and time, which is 06/05/2025 at 3PM. If no qualifed bids are received, ODH reserves the right to post Round 2 to allow more time for qualified bidders to submit proposals.

The Offeror must title the electronic files with “RDQ #SRC0000031104/DOH57310 - Technical Proposal” and "RDQ #SRC0000031104/DOH57310-Cost Form_Proposal” as appropriate.

Please note there is not a page minimum or maximum for the technical proposal and bidders should refer to technical evaluation criterion on the RDQ document for guidance.  The cost proposal should include the completed Excel Cost Proposal Form.
