obtained from the City of Dublin, Purchasing Department, P.O. Box 690, or 702 East

Madison Street, Dublin, Georgia.

and specifications.

The cost for a set of plans and specifications is $150.00 by a check made payable to

the City of Dublin.

Be sure to submit this signed form with your bid to acknowledge your receipt of

the addendum. Failure to return signed addendum may result in disqualification

of bid.
