Solicitation Summary
MHCC is seeking a contractor who can successfully deliver health care related actuarial services to assist the Commission in completing a financial impact analysis of requiring health insurers, nonprofit health service plans, and health maintenance organizations to provide comprehensive insurance coverage for advanced primary care management (APCM) services. Specifically, APCM services are defined the following codes:

G0556: This code is for patients with one or fewer chronic conditions

G0557: This code is for patients with two or more chronic conditions; and
-
G0558: This code is for patients with two or more chronic conditions who are also Qualified Medicare Beneficiaries.
