1 Proposal instructions and evaluation
Scope and proposal organization
Provide a complete ambulance and the required medical equipment for a single total delivered price. Include
all specified installation, wrap, reflective graphics, accessories, freight, preparation, testing, documentation,
and training. The City intends to purchase one ambulance; proposers with more than one available unit may
submit a separate, complete offer for each identified unit.
Organize the proposal as follows: signed response and pricing form; production and delivery evidence;
specifications and drawings; compliance and exceptions; warranty and service information; and
qualifications and references. Proposals, including the offered price and availability of the identified unit,
must remain valid for at least 45 calendar days from the RFP issue date of October 2, 2026.
Required production and delivery evidence
Identify the ambulance manufacturer, conversion model and year, chassis make/model/year, VIN if
assigned, manufacturer production or stock number, current location, and current stage of completion.
Include dated photographs and a manufacturer statement confirming that the offered unit is completed or
actively in production at the proposal deadline. A future production slot, proposed build, or unallocated
chassis does not satisfy this requirement.
Provide a schedule for remaining work, equipment procurement, graphics, testing, and delivery. Identify any
dependency on City-supplied equipment, any deadline for award needed to meet delivery, and the date
through which the unit will remain available. Confirm availability of both medical devices separately. An
estimated delivery month alone is insufficient; provide a firm committed delivery date.
Compliance and alternatives
Respond to every numbered requirement in Sections 2 through 5, using "Complies," "Exception," or "Not
included," with supporting specifications and page references. List every exception and its effect on safety,
price, service, and delivery. Silence does not establish compliance. Submit dimensioned floor plans, storage
layouts, seating and restraint details, and equipment mounting information.
Named ambulance conversion components establish the requested quality and functionality; documented
equivalents may be proposed. The requested ambulance is comparable to a high-roof Osage Travois
conversion. ZOLL X Series and LUCAS 3 are required medical product families. Identify the exact
model/version offered. Substitutions outside these required medical product families are not permitted.
Evaluation and award
The City will first review production status, the delivery commitment, required equipment, and material
specification compliance. Proposals that do not satisfy those requirements may be rejected. Responsive
proposals will be evaluated on total delivered cost, documented delivery reliability, quality and operational
suitability, warranty and local service support, and vendor experience and references. No numerical
weighting is assigned.
Provide three recent ambulance customer references with contact information. Identify the authorized
dealer, manufacturer relationship, dealer licensing/authorization to sell and deliver the offered vehicle in
Kansas, and nearest authorized service locations. Include current applicable licenses and describe chassis
manufacturer approval or qualification of the ambulance converter. The City may inspect the offered unit,
verify production directly with the manufacturer, and request clarifications or a demonstration. No addenda
will be issued; oral statements and responses to questions do not modify the RFP.
The proposer must be legally authorized to sell and deliver the offered motor vehicle in Kansas
and must submit the applicable dealer license and authorization documentation. Proposals that do
not satisfy this requirement will be rejected.

2 Chassis and patient compartment specifications
Chassis and conversion
2.1 One new, unused Type II ambulance with a high-roof Ford Transit T350 AWD chassis, 3.5L EcoBoost
engine, or documented equivalent. A new 2026 or 2027 chassis is acceptable; identify actual chassis and
conversion model years independently. No remount, used ambulance, or previously placed-in-service unit is
acceptable. Disclose all demonstration use, mileage, and warranty start dates.
2.2 Provide chassis specifications, GVWR, axle ratings, wheelbase, overall dimensions, interior headroom
and length, engine and transmission, fuel capacity, and installed chassis safety features. Provide extended
running boards on the driver and passenger sides. Identify their dimensions and coverage on the proposed
exterior layout.
2.3 The completed ambulance must meet all applicable federal KKK-A-1822E and Kansas requirements at
delivery. Identify the ambulance construction standard and edition used and provide supporting certification
and test documentation. Include all required Kansas ambulance inspection paperwork and supporting
documentation upon delivery. Identify any exceptions; a general brochure statement is insufficient.
2.4 Provide a completed-vehicle weight report and remaining payload calculation allowing for crew, patient,
City-supplied cot, installed loading and mounting equipment, oxygen, medical devices, and anticipated
supplies. State the assumed load allowance. The fully loaded vehicle must remain within GVWR and each
axle rating.
Interior layout and finishes
2.5 Provide a functional ALS treatment layout with street-side cabinetry and shelves, an action area,
curbside squad bench with storage, an attendant seat, overhead curbside storage, and a custom ALS
equipment stack with two pull-out drawers and a refrigerated drawer. Submit dimensions and access
clearances for City review.
2.6 Provide dove gray laminate, cobalt upholstery, and sapphire Lon-Coin flooring, or documented
equivalents. Interior surfaces must be durable, cleanable, and suitable for EMS service. Include cabinet
restocking seals, approximately ten stainless Southco-type latch upgrades, rear-door trim, squad-bench lid
gas struts, and cargo nets for the bench/storage area and ALS stack.
2.7 Provide an EVS three-point child-capable attendant seat or documented equivalent, with approved
base, restraint, and anchorage. Identify the number of belted seating positions and applicable
seating/restraint test documentation. Provide a stainless 12-inch grab bar, ceiling grab support, recessed
swing-down IV hanger, and a backboard storage slot approximately 18 by 5 inches, or disclose the offered
dimensions.
2.8 Provide three glove-box holders: two at the rear near the spare oxygen location and one at the upper
left storage area. Include an additional interior handle on the factory sliding door and lighting at the step well.
Oxygen suction and cot interface
2.9 Include one M-size oxygen cylinder in the base price, with transverse storage, a slide-out bracket, and a
compatible regulator. Secure all cylinders and provide accessible shutoff and regulation. Include a patient-
area oxygen outlet at the squad-bench wall in place of the action-area outlet; identify all outlet locations and
quantities. Provide two oxygen flowmeters with Ohio-compatible fittings. The City seeks a separately priced
option for secure storage of a second M-size cylinder, if feasible, to support long-distance transfers and
ventilator patients. Describe the proposed arrangement and price the second cylinder separately from its
storage hardware. If additional storage is not feasible, explain the limitation and identify any available
alternative.
2.10 Provide a functioning installed suction system suitable for ALS service. Identify suction equipment,
controls, and locations. Provide standard patient compartment heating, air conditioning, ventilation, and
lighting; identify system capacity and controls.
City of Independence KS | Ambulance RFP | Page 3

2.11 Cot Mounting and Loading System: The City will supply the existing Stryker powered cot from the
previous Medic 2. After award and before installation, the successful proposer and the City shall jointly verify
the cot model and compatibility to determine the proper loading, mounting, and release system to be
installed. The ambulance builder shall furnish and install new Stryker mounting racks and a new cot release
system compatible with the City's cot, with manufacturer-approved structural anchorage. Mounting racks
and release equipment from the previous Medic 2 shall not be reused. Identify the proposed loading system
and all components included in the base price. Include required hardware, installation, and operational
testing. A new cot is not included in this purchase. State the cot information needed, the required date for
delivery of the City-supplied cot, and any removal or transportation charges separately. Any change to the
awarded equipment or price resulting from compatibility verification requires prior written City approval.
3 Electrical warning systems and graphics
Electrical and communications
3.1 Provide an auxiliary electrical system appropriate for ambulance service, with protected circuits, labeled
wiring, suitable batteries/charging, and accessible service points. Submit electrical load information
demonstrating capacity for warning systems, HVAC, refrigeration, medical devices, and the cot-loading
system.
3.2 Provide a Vanner LSC12-1100 inverter/conditioner with remote switch or documented equivalent; one
20-amp automatic-eject shoreline connection and shoreline indicator; and an AC/USB outlet at the action
area. Include necessary supply cords and state connector requirements.
3.3 Provide dual USB outlets at the front console and upgrade one additional 12V outlet to USB-C. Include
a checkout-light timer, patient compartment digital clock, and digital thermostat.
3.4 Provide three total antenna prewires and three NMO mounts, with power and ground terminating behind
the driver's seat. Include ignition-controlled radio power and ground prewire for the patient compartment.
Identify cable type and termination points. Radio equipment is City-supplied unless specifically priced as an
option.
3.5 Provide a Dometic CD-20 refrigerated drawer or documented equivalent at the ALS stack, secured for
transport. Provide an interior patient camera with a color monitor in the cab, comparable to a Brigade
system. State whether the chassis includes a separate rear-view camera.
Emergency warning equipment
3.6 Provide LED warning lights visible around the vehicle. Include a six-light front pod arrangement
comparable to Whelen 700-series LEDs, with red/blue/clear/clear/red/blue positions. Equivalent
configurations must demonstrate comparable coverage and visibility.
3.7 Include two grille lights, one red/clear and one blue/clear; red/clear fender lights; blue/clear lights over
the wheels; and three blue rear/body warning lights comparable to Whelen 900-series lights in place of
standard red fixtures. Identify all light locations, quantities, models, and colors on the proposed exterior
layout.
3.8 Provide a Whelen 295HFSC9 dual-tone siren with a 200-watt output or documented equivalent, with
compatible speaker equipment and cab controls. Provide scene and loading lights and identify the warning-
light flash patterns. Final patterns and color configuration are subject to City approval and applicable
requirements.
Exterior finish and identification
3.9 Include red and black wrap and reflective graphics generally matching the City's supplied concept. Use
red PPG FBCH1002856 and black PPG FBCH911601, or approved color matches. Provide a black upper
roof area, red body, black side accent bands, and rear diagonal red/black reflective chevrons. Disclose any
stock-unit finish limitations.
3.10 Include "INDEPENDENCE FIRE-EMS" side and rear lettering and "INDEPENDENCE" front
identification in a College Block style or approved equivalent; include the Fire-EMS insignia on the front
doors. The City will supply approved logo/insignia artwork. Use 3M 780 reflective material or documented
City of Independence KS | Ambulance RFP | Page 4

equivalent. Submit a final scaled proof for approval before application; do not use the City's document-
header logo in place of the vehicle insignia.
3.11 Include two secured 5-pound fire extinguishers. Price optional wet sanding and buffing separately; this
finish option must not be included in the base total unless the City selects it.
Exhibit A summarizes the appearance concept. Any variation in layout, color, lettering, reflective coverage,
warning equipment, or graphics must be disclosed and accepted by the City before production changes are
made.
4 Required medical equipment
Supply the following devices as new equipment purchased through authorized channels. Include all
specified accessories in the base proposal. Identify exact catalog numbers, software options, quantities,
warranties, and any recurring charges. Refurbished, recertified, or demonstration devices are not
acceptable.
ZOLL monitor and defibrillator
4.1 One ZOLL X Series monitor/defibrillator, including an X Series Advanced model if offered, configured for
ALS field use. Identify the exact model and configuration. Include manual and AED defibrillation,
synchronized cardioversion, noninvasive pacing, ECG monitoring with 12-lead acquisition, pulse oximetry,
noninvasive blood pressure, and waveform end-tidal CO2 monitoring.
4.2 Include the ECG/12-lead cables, therapy cable, adult and pediatric defibrillation/CPR electrode starter
supplies, reusable adult and pediatric SpO2 sensors, adult/large-adult/pediatric blood pressure cuffs and
hose, and adult and pediatric capnography starter supplies compatible with the proposed configuration.
Itemize starter quantities and every accessory; "with accessories" alone is insufficient.
4.3 Include at least one installed rechargeable battery, one spare rechargeable battery, AC power/charging
accessories, carrying case, and all components needed to operate each required function. Identify any
separate battery charger. State data transfer capabilities, compatibility requirements, and included
software/licenses; separately identify optional subscriptions or software beyond basic operation.
4.4 Include a vehicle retention mount designed and tested for the proposed monitor, with installed structural
anchorage, preserving access to the patient and device controls. Provide mounting location, clearance
drawings, and supporting retention-test documentation.
LUCAS chest compression system
4.5 One LUCAS 3 chest compression system. Identify the exact version, including whether version 3.1 is
offered. Include the back plate, patient straps, stabilization strap, carrying case, and all standard operating
components and supplied suction cups required for immediate use.
4.6 Include one operating rechargeable battery plus one spare rechargeable battery, a compatible external
battery charger, external power supply, and applicable power cords. State charging requirements and
provide a parts list. Include a secured storage location or retention system appropriate for ambulance
transport without obstructing access or egress.
4.7 Identify device configuration and data connectivity capabilities and all included software, licenses, and
recurring fees. Configure the device to City-approved settings within manufacturer instructions. Do not
include an ongoing service subscription without separate disclosure.
Medical equipment support
4.8 Provide manufacturer instructions, current authorized warranty coverage, serial-number inventory, initial
functional checks, and hands-on orientation for both devices. Identify the authorized equipment supplier and
warranty service contacts. Provide the devices with the completed ambulance by the required delivery date.
The City will supply its remaining routine medical inventory and consumables, except those expressly
included above. Proposers must list any excluded item needed to operate the required devices. No
accessory substitution or equipment backorder may postpone delivery beyond January 8, 2027, unless an
alternate delivery date is approved in writing by the City.

standard vendor packages.
5 Delivery acceptance warranty and purchase terms
Delivery commitment and inspection
5.1 Deliver the completed ambulance and both required medical devices to Independence Fire-EMS, 120
N. 6th Street, Independence, KS 67301, during the week of January 4, 2027, defined as January 4-8, 2027,
unless an alternate delivery date is approved in writing by the City. Arrange delivery and City inspection
during an agreed staffed period. Shipping, factory completion, or an incomplete vehicle does not satisfy
delivery.
5.2 Include freight, transit insurance, preparation, and all required installations and graphics in the delivered
price. The vendor bears responsibility for the unit until delivery and City acceptance. Provide progress
reports at least every two weeks after award and immediate written notice of any event threatening the
delivery date.
5.3 The City will inspect the chassis and conversion; verify listed equipment, weight documentation, oxygen
and suction operation, electrical and HVAC systems, emergency lights/siren, restraints, mounts, cot-loader
operation, graphics, and medical-device operation. Correct deficiencies at vendor expense. Delivery receipt
does not constitute final acceptance of deficient work.
5.4 Provide clean ownership documentation and manufacturer certificate of origin or other appropriate title
documentation, VIN and serial-number inventory, warranty registration, operation/service manuals, wiring
diagrams, maintenance schedules, certification/test records, and installed equipment instructions. Provide
operator and maintenance orientation at delivery.
5.5 Manufacturer Inspection Visit: Include in the base price transportation, lodging, and meals for two City
representatives to visit the manufacturer's facility after award and before completion and delivery. The visit is
for inspection and final City approvals of the layout, equipment placement, graphics, and other details
required to finish the unit in accordance with the awarded proposal and RFP specifications. Identify the
travel arrangements and expenses included. Any change affecting price, specifications, or delivery requires
prior written City approval.
Warranty and service
5.6 Include the full new-chassis manufacturer warranty and a conversion warranty of at least three years or
36,000 miles, whichever occurs first, for conversion manufacturer workmanship/materials. Include an
ambulance wiring/electrical installation warranty of at least six years or 72,000 miles, whichever occurs first,
and graphics coverage of at least three years or 36,000 miles. Separately identify component manufacturer
warranties and all exclusions.
5.7 Provide written ZOLL and LUCAS manufacturer warranty terms, including duration, parts, labor, battery
coverage, and service arrangements. Warranties must begin at City acceptance; disclose any manufacturer
limitation or coverage already consumed by stock storage or demonstration use as an exception. Disclose
travel, freight, transport, and other costs the City would pay for warranty service.
5.8 Identify authorized chassis, conversion, and device service locations; parts availability; and emergency
support contacts. Respond to warranty service requests within one business day with a repair plan. Provide
optional extended warranties or maintenance agreements separately, with term, coverage, and price.
Pricing and contract terms
5.9 Provide a firm fixed total delivered price, including chassis, conversion, required options, wrap and
reflective graphics, devices, installation, training, and documentation. Do not include unilateral chassis-price
escalation or unpriced contingencies. List any applicable taxes separately and state the basis; the City will
provide appropriate exemption documentation.
5.10 The City's proposed payment term is payment after delivery, inspection, written acceptance, and
receipt of a proper invoice. Disclose any requested deposit, progress payment, or factory-completion
payment as an exception, with amount and safeguards. No such term is accepted by issuance of this RFP.
City of Independence KS | Ambulance RFP | Page 6

5.11 No award is made by this solicitation. The final purchase agreement will incorporate the accepted
proposal, RFP specifications, delivery commitment, and negotiated terms, subject to City approval. No
substitutions or price changes are permitted without written City agreement. The City may address failure to
deliver or cure material deficiencies through remedies stated in the executed agreement.
Proposers are responsible for their proposal costs. Identify any information claimed confidential and the
basis for that claim; blanket confidentiality statements will not bind the City. Disclosure will be governed by
applicable public records requirements.
