The Indian Health Service, Nashville, TN Area intends to make a sole source award (firm-fixed price) to Niagara Falls Memorial, 621 10th St, Niagara Falls, NY 14301.





In response to the diabetes epidemic among American Indians and Alaska Natives, Congress established the SDPI grant programs in 1997. This annual grant program, coordinated by IHS Division of Diabetes with guidance from the Tribal Leaders Diabetes Committee, provides funds for diabetes treatment and prevention to IHS, Tribal, and Urban Indian health programs across the United States.





This is a new grant funded project for Lockport Service Unit. The request for sole source for Niagara Falls Memorial Hospital to administer the scope of work as they currently hold the fiduciary responsibility for the Tuscarora Nation clinic. Services include Clinical RN diabetes management and outreach education and prevention activities. Project title is Special Diabetes Program for Indians (SDPI) grant for Tuscarora Nation and IHS Lockport Service Unit.





The Special Diabetes Program for Indians (SDPI) grant for the Lockport Service Unit shall allow diabetes education and prevention services to the Tuscarora and Tonawanda Tribal members. P.L. 110-275, Sec 303/25 U.S.C.13/PHS Act 330C.





Nature of the acquisition and proposed unique qualifications of the contractor(s). The need to hire Tribal members to perform the activities, diabetes management and reporting outcomes. No other source can offer the specific community and cultural capacity to reach grant participants than Niagara Falls Memorial, as they are the only ones currently allowed on the territory.





The period of performance for the award is anticipated to be a twelve month base period. 08/01/2025 (or date of award) – 07/31/2026 with three option years.





NO SOLICITATION IS AVAILABLE. A request for more information, or a copy of the solicitation, will not be considered an affirmative response to this Notice. Telephone responses or inquiries to this Notice will not be accepted. The Indian Health Service/Nashville Area will consider written responses received no later than 7/25/2025. Responses must be submitted electronically to donna.harjo@ihs.gov. Responses must include sufficient evidence that clearly shows your organization is capable of providing the content described in this notice. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no written response is received by the aforementioned deadline that clearly demonstrates an ability to meet all requirements, and that is more advantageous to the Government, Indian Health Service will make award on a sole source basis to Niagara Falls Memorial.

  2. UEI number.

  3. Specific details of the service(s) that your organization’s offers in regards to this notice.

  4. A brief capabilities statement (not to exceed 5 pages) that includes a description of your organization’s standard line of services.

No other information regarding this Notice will be provided at this time.





The period of performance for this service is to be a base year plus three option years. Indian Health Service/Nashville Area may consider information received no later than 11:00 am CST on Friday July 25, 2025 submit to donna.harjo@ihs.gov.
