The contractor shall provide all labor, equipment, supplies, materials, parts, services, permits, and licenses necessary to perform industrial water treatment at the Phoenix Indian Medical Center. The contractor must be registered with the State of Arizona and personnel performing the work must be licensed to perform work on the equipment specified in this statement of work in accordance with industry practice.
