|
James City County, Virginia (“JCC” or “the County”), on behalf of the Social Services Department is soliciting proposals for a software solution that will support our Department of Social Services team in managing an innovative child and family support program. The Child Health Initiative is a partnership between the local early childhood system, the PreK-12 school system, the local community action agency, and the local human/social services systems (in James City County and the City of Williamsburg) designed to help children in families with multiple health and socio- economic challenges achieve better health and wellbeing. Families are referred into the service delivery program, known as the “Care Team,” through the partner agencies, as well as through direct outreach/public awareness campaigns that lead to families self-referring directly to the program.
One of the main drivers of this solicitation of data solution proposals is the need to have web-based automated case management capacity so that Care Team members can focus more on building relationships with, serving, and connecting families to supportive services and focus less on data entry as a separate, time-intensive task. The proposed data solution will provide an opportunity to increase the efficacy of partnerships with families by infusing the care team’s data tracking tasks seamlessly into the case management process in an on-demand way (web-based) that travels with care team members with access to any Internet-connected device. Ideally, this would include the ability to upload photos, audio recordings, and real-time data entry. Automated workflow supported by the data solution also will increase the efficacy of the Care Team’s partnership with families.
Another factor prompting this RFP is the need to migrate six years of program data into a modern interface that would provide clear methods for program evaluation and maximum operational efficiencies. This could therefore transform program data into more actionable information for a variety of purposes, including the possibility of transitioning the Care Team from a promising practice to an evidence-based model.
