Summary

The St. Louis Area Agency on Aging (SLAAA), through the City of St. Louis Department of Human Services, is seeking proposals for a pilot program to expand services for family caregivers and evidence-based disease prevention and health promotion.

Program Overview:
Purpose: To provide short-term expanded services to family caregivers and implement evidence-based programs proven to improve health and reduce disease among older adults.
Target Population: Low-income minority populations and those with the greatest social and economic need within the service area.1
Funding: Up to $250,000 is available for Family Caregiver Services and up to $80,000 for Evidence-Based Disease Prevention and Health Promotion (DPHP).
Contract Period: Tentatively starting July 1, 2026, with all funds required to be utilized by September 30, 2026.

Scope of Services:
Family Caregiver Services: Includes home repair, case management, training, supplemental services, and in-home respite or personal care.
DPHP Programs: Must be evidence-based and published in peer-reviewed journals, such as "A Matter of Balance," "Tai Chi for Arthritis," or "Chronic Disease Self-Management".
Compliance: All services must align with Title III-D and Title III-E of the Older Americans Act.

Submission Requirements:
Deadline: Proposals must be received by July 8, 2026, at 4:30 PM CT.
Format: Preferred submission is via email in PDF format to stoevera@stlouis-mo.gov , though mail is also accepted.
Required Documents: Proposals must include a Bid Cover Sheet, RFP Checklist, Operational Plan/Scope of Work, Bid Submission Sheets for each service, and verification of licenses and taxes.
