(ANNUAL CONTRACT); RFB No. 26-0024

Use the form provided in the solicitation to acknowledge receipt of this addendum.

Failure to do so may result in your submittal being deemed non-responsive and not

receiving further consideration for award.

Vendors are informed that the above subject solicitation is hereby modified, corrected, or

supplemented as specified, described and set forth in this Addendum:

I. DUE DATE EXTESNION

The due date is extended; sealed bids are due no later than 2:30 PM on Wednesday, March

4, 2026. Bids that were submitted will remain sealed until the extended due date.

II. QUESTION(S)/RESPONSE(S)

Question 1: Should we enter a separate discount on each category that we supply or provide

a single discount range on the one-line form to cover the categories that we

provide?

Response: If a vendor wishes to provide discounts by category, please use Form 1A

(attached). If not, please use Form 1 to provide overall discounts.

III. ACKNOWLEDGEMENT

Indicate that your company has received this Addendum on the appropriate form and

includes with electronic response. Failure to acknowledge receipt of this addendum

may render your response “Incomplete”.
