Subject: Custom Fecal Collection Mailer Kits, Reagents, Consumables, Lab Equipment Maintenance and Training Support for The Defense Health Agency (DHA)
THIS IS NOT A REQUEST FOR PROPOSALS (RFP) OR A REQUEST FOR QUOTATIONS (RFQ); IT IS STRICTLY A REQUEST FOR INFORMATION (RFI). NEITHER UNSOLICITED PROPOSALS NOR ANY OTHER KINDS OF OFFERS WILL BE CONSIDERED IN RESPONSE TO THIS RFI. NO CONTRACT WILL BE AWARDED PURSUANT TO THIS ANNOUNCEMENT.
1.0 DISCLAIMER: This RFI is issued solely for information and planning purposes and does not constitute a solicitation. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. All information received in response to this RFI that is marked Proprietary will be handled accordingly. Responses to the RFI will not be returned. At this time, questions concerning the composition and requirements for a future RFP will not be entertained.
2.0 SUBJECT: The DHA is obtaining information for a potential materials and supplies contract with a minor non-personal services element, to support a Custom Fecal Collection Mailer Kit Program. This contract will also provide Reagents, Consumables, and other test resulting equipment maintenance and support to the Department of Defense (DoD) DHA Fecal Immunochemical Test (FIT) Kit Mailing program.
3.0 BACKGROUND: The DHA is seeking sources to provide support for the DoD DHA FIT kit mailing program. This program includes a systematic mailing program to large numbers of people (approximately 140,000 tests sent annually), with all tests mailed back to a single Department of Defense (DoD) lab, for resulting. FIT kits will be readied for mailing to patients and include mail specific requirements such as a pre-printed return envelope.
A Polymedco OC-Auto Sensor Diana FIT kit analyzer was purchased, along with an initial 35,000 tests sent to patients last year. Related requirements such as installation, training, integration with the electronic health record, and maintenance, was completed with a single DoD lab.
This potential contract is for on-going requirements to include:

d. Point of contact name, phone number, and email for individual with the authority and knowledge to clarify responses with Government representatives.
