Subject: Custom Fecal Collection Mailer Kits, Reagents, Consumables, Lab Equipment Maintenance and Training Support for The Defense Health Agency (DHA)

THIS IS NOT A REQUEST FOR PROPOSALS (RFP) OR A REQUEST FOR QUOTATIONS (RFQ); IT IS STRICTLY A REQUEST FOR INFORMATION (RFI). NEITHER UNSOLICITED PROPOSALS NOR ANY OTHER KINDS OF OFFERS WILL BE CONSIDERED IN RESPONSE TO THIS RFI. NO CONTRACT WILL BE AWARDED PURSUANT TO THIS ANNOUNCEMENT.

1.0 DISCLAIMER: This RFI is issued solely for information and planning purposes and does not constitute a solicitation. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. All information received in response to this RFI that is marked Proprietary will be handled accordingly. Responses to the RFI will not be returned. At this time, questions concerning the composition and requirements for a future RFP will not be entertained.

2.0 SUBJECT: The DHA is obtaining information for a potential materials and supplies contract with a minor non-personal services element, to support a Custom Fecal Collection Mailer Kit Program. This contract will also provide Reagents, Consumables, and other test resulting equipment maintenance and support to the Department of Defense (DoD) DHA Fecal Immunochemical Test (FIT) Kit Mailing program.

3.0 BACKGROUND: The DHA is seeking sources to provide support for the DoD DHA FIT kit mailing program. This program includes a systematic mailing program to large numbers of people (approximately 140,000 tests sent annually), with all tests mailed back to a single Department of Defense (DoD) lab, for resulting. FIT kits will be readied for mailing to patients and include mail specific requirements such as a pre-printed return envelope.

A Polymedco OC-Auto Sensor Diana FIT kit analyzer was purchased, along with an initial 35,000 tests sent to patients last year. Related requirements such as installation, training, integration with the electronic health record, and maintenance, was completed with a single DoD lab.

This potential contract is for on-going requirements to include:

• FIT Kits for mailing of approximately 140,000 kits, including return envelopes to be mailed by USPS Ground Advantage, shipped to a third party (the Government Publishing Office) on demand,


• Enough reagent and other resulting requirements to be used with the previously purchased automated analyzer for approximately 70,000 kits to be shipped to the DoD lab upon request.

4.0 RESPONSE INSTRUCTIONS: Respondents should address the following in their response:

4.1 Organization Information:

a. Organization’s full legal name and Address

b. Unique Entity Identifier (UEI), Cage Code, Tax Identification Number, and company structure (Corporation, LLC, partnership, joint venture, etc). Companies must be registered in the System for Award Management (SAM, at www.sam.gov) to be considered as potential sources.

c. Small Business Designation certified by the Small Business Administration, if any.

d. Point of contact name, phone number, and email for individual with the authority and knowledge to clarify responses with Government representatives.

Provide sufficient information/details to answer the following questions:

1. Does your company currently create and provide custom fecal collection mailer kits?


2. Does your company have the ability to provide fecal collection kits, reagents, supplies, and consumables that are compatible with the Polymedco OC-Auto Sensor Diana FIT kit analyzer?


3. Can your company provide 170,000+ Polymedco FIT kits annually in lots not to exceed 35,000 kits at a time, delivered to the Government Printing office?


4. Does your company have the ability to provide warranty, maintenance, support, diagnostics and training for the Polymedco OC-Auto Sensor Diana FIT kit analyzer?


5. Is your company an authorized dealer and or an authorized service provider for Polymedco?

Due Date: RFI response submission is due no later than 10 AM on 09 June 2025.

Page Limit: Limit your total written response to 25 single-sided pages. The cover page and table of contents does not count towards the page limit. Beyond the 25-page limit, the Government may only review examples of media and graphic design, as requested above.
