All examinations must be performed by a licensed optometrist or ophthalmologist. The
selected contractor shall provide an optometrist or ophthalmologist that will perform primary and specialty eye and vision care services and/or procedures. A prescription form for any glasses, if needed, must be completed by the licensed optometrist or ophthalmologist at the time of the examination and given to the Nursing Clerical office. The selected contractor will provide frames, lenses (single, bifocal or trifocal) and regular tint for lens glare, and diopters SPH#4, CYL2. The selected contractor must bring their own examination equipment |
