Summary

**TERM CONTRACT REQUEST FOR 2 DIFFERENT ITEMS ON SAME CONTRACT** These items are usually ordered once per year, average annual usage for School Bus Safety Inspection Decals is 55,000, Motor Vehicle Inspection decals 75,000 School Bus Safety Inspection Decals 7" x 7", decal, prints 4 PMS colors, Pantone 141, Pantone 032, Black, and White represented by pink, one side on 2 mil clear mylar with serialized numbering, and permanent adhesive and split back liner prints black ink. Shrinkwrap 100 per package. OSHP 2027 Expiration Motor Vehicle Inspection Decal. Prints two-sided, with full bleeds. Front-side prints, two color (Silver Gray and Black), with black, serialized numbering. Back-side prints, three color (Pantone 032C, White, and Black), in fade resistant inks. Decal is 2-7/8" x 3-1/4", 3 mil clear, non-yellowing acrylic-security permanent adhesive, with release liner. Booklet cover prints one-sided, on 110# white cover, serialized range prints bottom middle in black ink. With 30pt chipboard backing, 20 decals/booklet, perforated, and stitched twice at the top. Carton pack @ 25 booklets/box.

STANDARD TERMS AND CONDITIONS: The State of Ohio Standard Terms and Conditions (revised 03/31/25, attached) apply to this solicitation and will govern any resulting Contract.

EVALUATION: Bids will be evaluated in accordance with ORC 125.07. In addition, the state will multiply the bid prices by the estimated usage listed in the bid and then adding each of the totals together to arrive at a total for all items.

CONTRACT AWARD: The contract will be awarded to the lowest responsive and responsible bidder by low lot total. The low lot total will be determined by multiplying the unit prices by the estimated usage listed in the bid and then adding each of the totals together to arrive at a total for all items. Failure to bid on all items may result in the bidder being deemed not responsive.
