DESCRIPTION: NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT TO Hologic Sale and Service LLC to supply one (1) brand name Hologic Thin Prep Reagents and consumables and its components to provide consistent, high quality cytology testing.
STATEMENT OF WORK
HOLOGIC T2000 THINPREP REAGENTS
BACKGROUND: The department of Pathology and Laboratory Medicine Service at JJP Bronx VA Medical Center requires a brand name (Hologic) only reagents and consumables for its owned T2000 THINPREP, serial number N0319H19D0. Hologic reagents are currently being purchased using contract number 36C24221D0045 that will expire on 12/09/2025. Hologic ready-to-use reagents are certified and tested for quality to provide consistent, high-quality cytology testing. Hologic reagents and consumables are the only reagents and consumables that can be used on the T2000 THINPREP analyzer.

SCOPE: This requirement is for the procurement of Hologic brand reagents and consumables for the T2000 THINPREP, serial number N0319H19D0. This action will eliminate delays and ensure continuity of service. All reagents and consumables orders will be placed directly to the contractor. The contractor shall submit all invoices through the Tungsten e-Invoice Network for review, certification, and payment.
Contractor shall: 1) Ship all reagents and consumables, with longer expiration dates, within 48 hours of request directly to JJP VA Medical Center, Monday through Friday, and 2) Provide customer support by phone 24 hours a day, 365 days a year to assist in trouble shooting.

No delivery shall be performed outside the scope of this contract. Any justification must be pre-approved by the Contracting Officer, and a modification of the contract as well as a purchase order must be issued before any request is honored. Failure to obtain prior authorization may result in non-payment of charges incurred.

PERIOD OF PERFORMANCE: Performance shall be for Base plus two years.

Any services not expressly stated in this Statement of Work are not authorized. No person, including the COR or any other VA personnel may request additional work be performed by the Contractor, except for the Contracting Officer. Any added services performed that are not authorized by the Contracting Officer will be considered inclusive.

This Notice of Intent is not a request for Proposals or Quotations. No solicitation document exists, and no telephone or email inquiries will be accepted. No contract award will be made based on proposals or quotations received in response to this notice.

Interested persons may identify their interests and capability to respond to the requirement. They should furnish detailed data concerning their abilities to provide services sought including pricing data and technical data sufficient enough to determine that a comparable source to the government is available.

The government may consider all Expressions of Interest received before November 5, 2025. A determination by the government not to complete this proposed contract based upon responses to this notice is solely within the government s discretion. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.

Responses to this notice must be emailed to Sanchez.Reid@va.gov and received by 4:00 PM EST on November 5, 2025. Submitters are responsible for ensuring their EOI reaches the designated contract specialist/contracting officer on time.
