Ohio Department of Health- DOH57307-Bureau of Maternal Child & Family Health-Youth Homelessness Data

ODH would like to contract with a vendor to enhance data collection for the Services for Homelessness Youth and Homeless Pregnant Youth (HY) grant. This contract will support data collection using Homeless Management Information System (HMIS) projects, creating reports and providing technical assistance on HMIS and data collection for ODH subrecipients.  In addition, vendor will provide technical assistance to ODH subrecipients related to youth homelessness.

Offerors will need to review the attached RDQ DOH57307- Youth Homeless Data for #SRC0000030576. Each Offeror must submit a technical proposal and cost form/proposal as part of its total Proposal before the closing date and time, which is 05/20/2025 at 3PM. If no qualifed bids are received, ODH reserves the right to post additional rounds to allow more time for qualified bidders to submit proposals.

The Offeror must title the electronic files with “DOH RDQ #SRC0000030576/DOH57307 -- Technical Proposal” and "DOH RDQ #SRC0000030576/DOH57307 - - Cost Form_Proposal” as appropriate.

Please note there is not a page minimum or maximum for the technical proposal and bidders should refer to technical evaluation criterion on the RDQ document for guidance.  The cost proposal should include the completed Excel Cost Proposal Form.
