Vendor Service Contract for Medical Direction and Physician Services

For this RFP, the City of Milwaukee is using a Bonfire portal for accepting and evaluating proposals digitally.

All required documents are available for download on the Bonfire portal.

Your submission must be uploaded, submitted, and finalized prior to the Closing Time of December 3, 2024 at 4:30 PM CT. We strongly recommend that you give yourself sufficient time and at least ONE (1) hour before Closing Time to begin the uploading process and to finalize your submission.

Important Notes:

The awarded Contractor will be required to submit proof of Insurarnce. Please refer to the Insurance Requirements section on the Invitation to Bid form.

A virtual pre-proposal informational meeting is scheduled for Wednesday, November 13, 2024, at 1:00 PM CT. Proposers who plan to attend the pre-proposal meeting must sign up in advance by Wednesday, November 6, 2024.
