for a term of one-time purchase, subject to contract performance and annual appropriation by the Raleigh

publicly opened for furnishing the apparatus, supplies, materials, equipment and/or repair work and services as
described herein.

and agrees, if this bid be accepted, to furnish and deliver any or all items upon which prices are bid, at the prices
set opposite each item within the time specified herein. By executing this bid, the undersigned Supplier certifies

certifies to the best of Supplier’s knowledge and belief, that it and its principals are not presently debarred,
