This Addendum consists of (4) page(s).

Acknowledge receipt of this Addendum by entering its number and date on the Proposal Form. This Addendum forms

a part of the Contract Documents and modifies them as follows:

1. Please use the attached Revised Bid Schedule for your bid submission.

2. Questions & Answers

1. As we are reviewing the bid documents, we noticed the numbers in the pricing template do not mirror up with

the 86 devices listed in the RFP (pages 3-6). For example, on the pricing template you have a total of 168

which doesn’t equate, even if figuring two inspections per year (86x2). Also, the Registration Fees are for a

total of 166 units which is not consistent. Can you please verify and explain how you came up with your

totals? We have updated the totals and are in the attached updated Price schedule.

a. Elevator Counts: The second column, which combines Year 1 and Year 2 pricing, contains an

incorrect count for elevators. I counted 16 cable elevators, which should result in a total of x32 rather

than x30. See updated Price Schedule

b. Pricing Categories: The State of Vermont separates hydraulic elevators into two distinct categories

for pricing. Additionally, platform lifts and stair climbers require separate pricing. I also noticed that

some of the totals in the second column are incorrect. See updated Price Schedule

c. Reinspections: These are dynamic, making it impossible to estimate the total number of elevators or

lifts that will require a reinspection for Year 1 and Year 2. While a fixed price per unit is acceptable, we

cannot accurately provide a total for those years. For re-inspections, provide a cost for one re-

inspection. The contract will carry a contingency based on that price times the average number of re-

inspections.

d. There should not be a registration fee section. Registration fees are collected when you register with

the Secretary of States Office as a company doing business in Vermont. Those fees cannot be waived.

The column on the price schedule was in error and has been removed.

Section 9.3: Fees: (a) An elevator inspector may charge a fee for an inspection required under these

rules. The fee shall include the $25 fee paid to the department for each authorized certificate of

operation and shall not exceed: (1) for electric traction elevators and escalators - $250, (2) for

hydraulic elevators greater than 3 stops - $225, (3) for hydraulic elevators with 2 or 3 stops - $200, (4)

for limited use/limited application elevators - $175, (5) for platform lifts - $150, (6) for residential

elevators and stairway chairlifts - $100.

3. It states the contract is for 5 years, yet it also asks for pricing for 2 years. How many years is this contract

for? The contract is for 5 years.

4. The State Elevator Board is currently updating its code, which includes a probable price increase. Since we

are regulated on what we can charge for inspections, predicting the increase is difficult.

If the board mandates a new price, will we be able to reflect that in the contract? If the board mandates a new

price, we will issue a change order to reflect the change in price accordingly.

5 Since reinspection are based on issued Conditional Certificates, will we receive a detailed list of units already

inspected to date, along with a list of the safety tests performed prior to inspections? Yes, upon award of the

contract.

6. Do "registration fees" refer to the stickers purchased from the board? As before, that price is currently $25 per

sticker, although it may increase soon. No. These fees are costs associated with registering as a business in

Vermont. That category was added to the price schedule in error. See updated price schedule. That category

has been removed.

7. To quote an accurate price schedule, will a detailed list showing When all units were last serviced and their

inspection expiration dates be available to us? All units were inspected in January of 2026.

8. The contract term is five years per Section 1.2, but the Price Schedule requests only Year 1 and Year 2

pricing. How will pricing be established for Years 3 through 5, and will the State allow an annual escalator for

those years? The contract will be for 5 years. Please provide pricing for years 1 through 5, on the updated Price

Schedule.

9. The inventory does not include certificate expiration or last-inspection dates. Will the State provide the

current due date and last-inspection date for each unit, and indicate how many units will be due or overdue at

the anticipated August 1 start date? We will provide that information, to awarded contractor.

10. Does "annual inspection" cover only the periodic no-load inspection, or are any five-year full-load tests

required during the contract term? For any load testing, will the State's maintenance vendor supply the

mechanic, rigging, and weights, or is that the inspection contractor's responsibility? The annual inspection

refers only to the annual fire safety inspection and any re-inspections of conditional certificates issues due to

violations needing to be addressed.

11. Section 2.17 limits subcontracting. If a bidder proposes to perform inspections using a named Vermont-

licensed inspector engaged on a partner or 1099 basis, will the State consider that subcontracting, and will it

approve subcontractors named in the proposal? The firm performing the inspections must have a Vermont

licensed inspector on staff. We will not accept partner or 1099 basis.

12. Can the State provide a reconciled equipment inventory, and confirm whether the Price Schedule quantities

(30 cable, 121 hydraulic, 17 electric lifts, 166 registrations) represent two-year totals? The listed inventory

totals approximately 86 units. See attached updated Price Schedule

13. What is the scope of the "Registration Fees" line item? Is it a State fee that the contractor passes through, or

a contractor administrative fee? Any fees levied by the Secretary of State or Department of Taxes are associated

with doing business in Vermont.

14. Please confirm the specific insurance limits required under Attachment C, Section 8, including any

professional liability or errors and omissions coverage. These requirements can be found by clicking through

15. Will the award be a single statewide contract, or may it be split by district, and how is cost weighted against

the other evaluation criteria in Section 3.6.1? This will be one statewide contract.
