Project Description: Issuing Procurement Unit: State of Utah Division of Purchasing
Conducting Procurement Unit: The Department of Health and Human Services. All divisions within the Department of Health and Human Services may use this contract.
The purpose of this RFP is to enter into a contract to provide services to restore eligible DHHS Adult Protective Services (APS) and other divisions' client homes to a safe, sanitary, and habitable condition by removing excess clutter, biohazards, and other debris..
It is anticipated that this RFP will result in multiple contract awards.
The methodology or formula that will be used to determine the number of contract awards is the vendors that receive a minimum score threshold of 840 Technical Points and a combined threshold of (Technical points + Cost Points) of 990 points will receive a contract award.
Length of Contract: 5 years
Vendor must guarantee its pricing for 1 year.
