Qualifications:

All offerors are required to hold all applicable South Dakota tax licenses. All proposals must be signed and submitted in a sealed envelope and shall state on the left-hand corner of the envelope “Proposal for Tree Removal Services.” Any proposals received after the deadline will not be accepted.

Miscellaneous:

Proposals may not be withdrawn within thirty (30) days after the actual opening thereof. Action on the proposals will occur within 30 days of the bid opening. The City and County of Brookings reserves the right to waive irregularities and to reject any and all proposals.

TO: THE CITY OF BROOKINGS, SOUTH DAKOTA
The undersigned offeror, having examined the Specifications and other contract documents and
addenda attached hereto and being acquainted with and fully understanding the requirements,
provisions, and conditions of this Project, proposes and agrees that Offeror will enter into and
perform a contract consistent with the attached specifications for the hourly price submitted on
this Proposal Form, and will do the work and furnish the equipment, transportation, and services
for said work in strict accordance with the Specifications, which form a part of the contract
documents; for the price shown on the following proposal items:
One contract will be awarded. The owner reserves the right to reject any and all proposals and to
waive any irregularities in the proposals received. All Offerors are required to have a South
Dakota Sales Tax Permit Number and must provide that number in the proposal.

set forth in these Contract Documents.

Brookings, South Dakota, will not be responsible for any errors or omissions on the part of the
undersigned Offeror in making this proposal.
