Solicitation Summary

The purpose of this contract is to establish a contract DHS/HCDSS for the provision of drug testing services for DHS/HCDSS. The drug testing services are intended to ensure a safe and drug-free policy, to comply with the applicable laws and regulations, and to support the DHS/HCDSS's drug-free programs.

Additional Instructions

The contract covers the following activities, tasks, and deliverables:

[Contractor Name] will provide drug testing services for employees or candidates (Parents, caregivers, Foster Youth) of DHS/HCDSS who are involved in HCDSS
Programs.

The drug testing services will include [Standard 16 panel: AMP - Amphetamines, BAR -Barbiturates, BUP - Buprenorphine, BZO - Benzodiazepines, COC - Cocaine, EDDP -Methadone Metabolite, ETG - Ethyl Glucuronide, FEN - Fentanyl, K2 - Synthetic Marijuana, MDMA - Ecstasy, MET - Methamphetamines, OPI / MOP - Morphine /
Opiates, OXY - Oxycodone, PCP - Phencyclidine, THC - Cannabinoid (Marijuana), TRA - Tramadol] testing using methods of, such as [urine, saliva, hair, blood, etc.] samples,
such as pre-employment, random, post-accident, reasonable suspicion, occasions [Contractor Name] will collect the samples from the employees or onsite, using
collection procedures, such as chain of custody, identification, verification protocols.

[Contractor Name] will document and report the results to DHS/HCDSS using reporting procedures, such as electronic and paper formats, within 1-3 days of analysis completion.

The results will include information to be reported, such as positive, negative, adulterated, substituted, invalid findings along with supporting documents such as chain of custody forms, laboratory reports.

[Contractor Name] will comply with the DHS/HCDSS 's drug-free policy, as well as the relevant federal, state, and local laws and regulations, such as [name of laws and regulations], regarding the drug testing services.

The following are the expected outcomes and deliverables of this SOW:

[Contractor Name] will deliver the drug testing services and supplies (oral swab kits, 10-16 panel instant read urine cups) in a timely, accurate, and professional manner, meeting the requirements and expectations of DHS/HCDSS.

[Contractor Name] will provide DHS/HCDSS with the results and reports of the drug testing services, along with the supporting documents and records, in a confidential and secure manner, respecting the privacy and rights of the employees or candidates.

[Contractor Name] will assist DHS/HCDSS in implementing and enforcing the drug-free policy, as well as complying with the applicable laws and regulations, regarding the drug
testing services.

Requirements

The requirements of this SOW are as follows:
