The purpose of this Invitation to Bid (ITB) is to select a Provider to provide monetary rebates to the Florida Department of Health’s (Department), Bureau of WIC Program Services (State WIC Agency), following a participant’s purchaseof infant formulafrom an authorized retail grocer, including formula issued to women and children via Food Package III. Bidders are required to specify a rebate and brand name for a single milk-based and soy-based primary contract formula for the following physical forms: concentrated liquid, powdered, and ready-to-feed.
