Solicitation Document Description: Watermain and Substandard Water Service Replacements on Queens Park Crescent East and Grosvenor Street; Bikeway Improvements on Queens Park Crescent East; Local Road Resurfacing on Grosvenor Street
Specific Conditions for Procurement: 1.        Experience Requirements
The Supplier must have the following experience in order to be considered for award of the Contract:
a.        Must have completed and must have achieved substantial performance (as defined by the Construction Act) for three (3) infrastructure projects that demonstrate linear infrastructure type work in an urban environment, including coordination with municipal operations and tie in with existing systems. The projects must contain mainline watermain work, where the watermain component of the work constitutes more than twenty percent of the contract value (or subcontract value) within the past five (5) years of the issuance date of this solicitation event.
i.        Two (2) of these three (3) projects must have a minimum contract value (contract value at the time of project award as defined by the Construction Act) of five million Canadian dollars ($5,000,000.00) each excluding taxes.
ii.        At least two (2) of these projects must be performed by the Supplier as a General Contractor.
iii.        At least one (1) of these projects must include watermain greater than 300 mm in diameter
iv.        At least one (1) of these projects must involve watermain rehabilitation by cured-in-place-pipe lining method.
b.        In addition to the above, Suppliers are to complete and upload Table 4C-1 in line item 4.3.3.1 in Part 4 – Submission Forms in SAP Ariba, providing project references demonstrating the completion of:
i.        at least 3,000 metres (either as a General Contractor or as a Sub-Contractor) total of watermain mainline installation (excluding large diameter services, hydrant leads, and other service pipes) within three (3) years of the issuance date of this Solicitation event.
ii.        Project examples in 1.a. may also be used for 1.b. for the purpose of determining the Supplier's total watermain mainline installation completed. Table 4C-1 may be reproduced but not modified. Suppliers may add rows as needed to demonstrate the requisite experience.
2.        Project Experience / Comparable Projects
a.        The Supplier must provide three (3) reference projects for the purpose of confirming the Supplier’s experience and track record of success. The Supplier shall provide the requested references for clients who have obtained construction services similar to those required for the Project from the Supplier in the last five (5) years. Failure to provide verifiable experience shall result in the bid being declared non-compliant.
3.        Qualification Requirements
a.        Current Certificate of Recognition (CORTM) or equivalent OHS Certification
Each Supplier shall acquire and upload a copy of a valid Certificate of Recognition (COR™) or Equivalent OHS Certification in the legal name of the Supplier or in the case of a joint Bid, for, and in the name of, each member of the Joint Bid Team.
b.        Contractor Qualified Person (QP) Qualifications
Each supplier shall complete this form as part of their submission. Provide the name and reference of who will be the Qualified Person as defined under O.Reg. 153/04 and O.Reg. 406/19 in Part 4 – Form F – Contractor QP Qualification and Experience.
i.        The Supplier shall identify and provide the following requirements for the Supplier's QP Personnel, in addition to meeting the definition of Qualified Person under O.Reg.153/04 and O.Reg.406/19:
1.        A minimum ten (10) years of work-related soil management experience for projects of similar size, scope, and complexity to the Work;
2.        A current licence in good standing with a public body within the province of Ontario such as Professional Engineers of Ontario or Professional Geoscientists of Ontario;
3.        Knowledge of Environmental Protection Act (Ontario), O. Reg. 153/04 and O. Reg. 406/19;
4.        Knowledge of relevant federal and Ontario environmental policies, procedures and legislation/regulations;
5.        List of the company name where this person has worked for in the past five (5) years;
6.        Experience liaising with other specialty consultants, contractors, agencies, and governmental authorities;
7.        Experience with similar projects in the province of Ontario related to excess soil;
Provide a copy of the resume or curriculum vitae containing at least two (2) project examples demonstrating the requirements above.
c.        Soil Management Sites (Class 1 – Class 2)
Each Supplier shall complete and upload this form as part of their submission. Pursuant to Section GN133.04.05 Approved Receiver Sites and Reuse Sites, the supplier shall be responsible for finding appropriate receiver sites and reuse sites to complete the project to according to O. Reg. 406/19. The supplier shall submit the list of receiver and reuse sites (including site addresses) and Class Type in the table in Part 4 – Form E Soil Management Sites (Class 1 – Class 2).
