Project Description: The purpose of this request for proposal is to enter into a contract with a qualified full service laboratory that can provide in-house substance use testing for multiple media types and test selections to provide drug analysis of youth who fall within the jurisdiction of the Fourth District Juvenile Court. Given the large geographical area of this judicial district, it is anticipated that this RFP will result in a contract award to one vendor per county. A vendor may be awarded any or all counties based on their scoring.
Please provide a proposal that addresses all of the requirements and scope of work, including the Evaluation score sheet categories from the attached RFP document.
Please provide a cost bid on the attached Cost sheet. Cost must be attached as a separate file from the technical proposal.
