Project Description: Issuing and Conducting Procurement Unit: Granite School District
The purpose of this Request for Proposals (RFP) is to enter into a contract to provide group basic life, basic accidental death & dismemberment (AD&D), voluntary supplemental life, and voluntary AD&D according to the attachment included in the documents section.
It is anticipated that this RFP will result in a single contract award to the responsive and responsible vendor with the highest score justified by the procurement code.
Contract Term: 3-year initial term with two one year options to renew.
