KPAC1811-2 to include all shipping and delivery fees to F.O.B. destination,
warranty, and all applicable fees and taxes, as follows:

Special Instructions

1. Quote shall include all shipping and delivery fees to F.O.B. destination, and all
applicable fees and taxes.

2. Vendors shall bid on all items in order to be eligible for award.

3. Items shall be new and as specified.

4. Vendor shall coordinate delivery with Kenji Kubo (808) 464-1350.

5. To be eligible for award, vendors must be compliant with the Internal Revenue Service,
State of Hawaii Department of Labor, State of Hawaii Department of Commerce and Consumer
Affairs, and the State of Hawaii Department of Taxation prior to the quote submission
deadline. The Hawaii Compliance Express (HCE) will be utilized to confirm vendor’s
compliance with all agencies. Information regarding HCE is available at
http://vendors.ehawaii.gov/hce/splash/welcome.html.

Offerors shall electronically register with HCE to obtain a Certificate of Vendor Compliance
which is acceptable as proof of compliance. The certification should be applied for in a
timely manner. The University will inform the successful offeror in writing as to the exact
date and time that the above certifications are due to the University. If the successful
offeror does not submit the certifications by the date and time specified in the
University\\\\\\\'s written notification, the successful offeror’s quote shall be rejected.
Thereafter, the University reserves the right to consider other offers received for award.

6. Item shall be delivered to the University of Hawaii at Hilo, 200 W. Kawili Street, Hilo,
Hawaii 96720.

7. The manufacturer and model number specified are used as a measure of quality and
performance. Any brand or manufacture of equal or better quality and performance to that
specified will be considered for acceptance by the University upon submission of
manufacturer\\\\\\\'s literature or brochures containing technical data and illustrations of the
proposed items being offered. Vendor shall include brochures with technical specifications
as an attachment to the SuperQuote RFQ submittal. Submittals with insufficient information
for evaluation shall be rejected. The University may reject or deny any proposed alternate
that it may, in its discretion deem unequal, and the findings in this regard shall be
accepted by the vendor as final and binding.

8. The vendor shall provide a minimum one (1) year warranty.

9. Unless otherwise agreed to, item shall be delivered no later than 5 consecutive calendar days
from the purchase order date.
