The purpose of this Request for Quote (RFQ) is to establish a contract or issue a purchase order with the responsive and responsible vendor that submits the lowest priced quote for the following items:
Note 1: Shirts are to have 3 color logo on back by neck, and to have the Block S logo (see pdf attachment) embedded/woven within the pattern of the shirt (custom fabric). Material to be 90% polyester and 10% spandex.
Note 2: Hats are to be gray with 5 panels with snap back. Mesh on back 2 panels, with a patch logo sewn on approx. 3"x2" with the block S and Snow College. Also, a small embrodier badger head on the right side of the hat just before the bill of the hat (see pdf attachment).

Additional Notes:
- Delivery must be FOB Destination to the specified location in Utah. All shipping and handling costs must be included in the quoted price.
Evaluation of Quotes: Each quote received shall be evaluated for responsiveness in as outlined in the Utah Procurement Code. This RFQ shall be evaluated by the objective criteria described in this RFQ. Quotes submitted must comply with the specification identified in this RFQ.
Multiple or alternate quotes will not be accepted. If a vendor submits multiple or alternate quotes, Snow College will only accept the bidder's primary bid and will not accept any other bids constituting multiple or alternate bids.
Snow College Purchasing may solicit other quotes via email, phone, or other methods as permitted by the Utah Procurement Code or applicable Administrative Rules.
