Quotation must include all applicable charges, taxes, shipping, etc. Quote shall be considered firm.

Bid price shall be valid for a minimum of 45 days.

Under Hawaii law, HRS 103D-310(c), you must provide proof of compliance in order to receive a contract worth $2,500 or more with state and county government entities in Hawaii. Vendors need to obtain a copy of their “Certificate of Vendor Compliance” via Hawaii Compliance Express. To register with Hawaii Compliance Express, go to https://vendors.ehawaii.gov/hce/splash/welcome.html. Vendor must provide proof of compliance 1 day after closing date.

ORDER THEM AS NEEDED THROUGHOUT THE 12 MONTH PERIOD.

The award of contract, if awarded, shall be made to the lowest responsive and responsible offeror on the TOTAL AMOUNT.

Please do not submit quotation unless able to quote on all items. No partial awards will be made.

NOTE TO OFFERORS

An acceptable quotation must conform in all material respects to this Request for Quotations. Any of the following may be
grounds for disqualifications:

1. Taking exception to any of the specifications, terms or conditions contained in the
RFQ.

2. Placing conditions on the furnishing of solicited goods or services.

3. Inclusion of a quotation or order form containing additional specifications, terms or conditions.

4. Referencing external documents containing additional specifications, terms or
conditions.

Offerors are advised that quotations are evaluated as submitted and requests by offerors to delete conditions contained in their quotations after quotation opening cannot be considered.
