REQUEST FOR STATEMENTS OF QUALIFICATIONS for PROFESSIONAL GENERAL CONTRACTING SERVICES
PROJECT ANNOUNCEMENT:  The Sarasota County Public Hospital District d/b/a Sarasota Memorial Health Care System and its governing board (collectively, the Hospital), located in Sarasota County, Florida, is accepting statements of qualifications from qualified construction management firms under the provisions of Section 255.103 and the Consultants Competitive Negotiation Act, Section 287.055, Florida Statutes.
PROJECT DESCRIPTION: The project involves an interior tenant infill and a small addition to a developerbuilt structure, specifically the first and second floors of a threestory medical office building located at 2695 Curry Lane, Nokomis, Florida. The first floor includes approximately 16,115 square feet of tenant infill for a new radiation oncology program featuring PET/CT, CT simulation, exam rooms, infusion spaces, and associated clinical support areas. In addition, the project includes a 2,570squarefoot addition designed to house two linear accelerators with the required vault construction. The second floor includes approximately 16,235 square feet dedicated to a gastroenterology outpatient unit, including endoscopy procedure rooms, prep and recovery areas, exam rooms, and sterile scope processing. The scope of professional general contracting services include, but are not limited to, pre-construction phase services such as cost estimating, value engineering, critical path method scheduling, constructability reviews and cost control, in addition to construction management services. Firms interested in being considered as candidates are required to submit five (5) bound statements of qualifications that include at least the following data, to be organized in the following order:
1. A copy of the Firms current applicable Florida construction licensure and corporate registration certificates.
2. Completed AIA Document A305 Contractors Qualification Statement, latest edition.
3. A description of any material regulatory actions involving the Firm within the past five (5) years, including the nature of the action, the regulatory agency involved, and the resolution or current status.
4. Proof of general, automobile and workers compensation liability insurance coverage.
5. A complete list of all the firms relevant insurance coverage statements that provide minimum coverage limits that provide proof that the minimum insurance coverage provided is sufficient to protect the Hospital for this project.
6. Proof of current bonding capacity and current usage of bonding capacity and available bonding capacity remaining.
7. A separate statement as to whether the firm is a certified Minority Business Enterprise or Woman or Minority Owned Business Enterprise (WMBE). If your firm claims MBE, WMBE status, a copy of your firms current, valid MBE/WMBE certificate is required as part of your qualification statement submittal.
8. A list of at least five client references consisting of name, title, organization, address, telephone number and project name(s) for each project specified. Please ensure your references are updated and willing to reply. Firms must ensure that all listed references are current, available and willing to reply to the Hospitals inquiries during the evaluation period.  No Hospital employees shall be used as references for purposes of this Request for Statements of Qualifications. Failure to provide five responsive references may result in the submittal being deemed non-responsive.
9. A summary of the Firms construction and permitting experience within the City of Venice, Florida and other applicable permitting agencies.
10. Resumes of key personnel that would be used on this project to include their project relevant experience.
11. A list of projects summarizing the Firms relevant experience with the construction of outpatient clinical facilities, radiation oncology centers, linear accelerator vault construction, endoscopy suites, or comparable healthcare projects similar in scope to the project described above.
12. Location of the firms main office, and proposed project team office location (if different from main).
13. An explanation of how the Firm intends to respond expeditiously on urgent project matters and a summary of the Firms approach to this particular project. 
14. Site construction management experience to include but not limited to: Structure, Roadway Access, Surface parking, Private Utilities, Public Utilities, Exterior Lighting, Landscaping and Drainage, and Retention.
Submissions must be complete and on time.  Incomplete or tardy submissions will be returned unopened and the responding Firm disqualified.  Submittals are part of the public record. Application materials will not be returned. 
The basis for selection criteria for this project includes, but is not limited to:
1. The Firms relevant professional general contracting experience in the past five (5) years as it relates to the above referenced project;
2. The Firms relevant professional general contracting experience with regulatory agencies with applicable jurisdictional authority, including, but not limited to, federal, state, and local agencies;
3. The Firms depth of construction management team and the resources available for this assignment.  This includes recent, current, and projected workloads of the Firm;
4. The location of the construction management firm's main / corporate office;
5. The Firms approach to this particular project;
6. The Firms ability to respond quickly;
7. Whether or not the lead Firm is an MBE / WMBE (valid certificate from the Office of Supplier Diversity required); and
8. The volume of previously awarded projects to the Firm by the Hospital.
9. The Firms available bonding capacity for this project.
It is the Hospitals responsibility to negotiate a fair, competitive and reasonable compensation per Section 287.055, Florida Statutes. A fair, competitive and reasonable compensation shall be evaluated based upon the following information: (1) Compensation on similar projects; (2) other compensation reference data; and (3) after approval of the ranking, proposals requested from the shortlisted firms based upon a scope of services document to be provided at the time of negotiations.
All interested Firms are further informed as follows:
1. The Hospital reserves the right to reject any or all submittals at any time during this process.
2. The Hospital reserves the right to request additional information beyond the data set forth above.
3. The Hospital reserves the right to waive any minor irregularities, informalities, or technical deficiencies in a submittal, provided that such waiver does not materially affect the qualifications of the responding firm.
