Ohio Department of Health-DOH56071-BMCFH-SFY25-Data for Adults with Type 2 Diabetes and Oral Health-RDQ

ODH is seeking to contract with a vendor who will collect data by accessing FQHCs’ electronic medical records using analytical and reporting software, such as Azara, and conduct a survey to collect data not contained in electronic medical records. The data provided to ODH will be analyzed and disseminated by the Oral Health Program with the overarching goal to increase access to integrated medical and dental services among adults with Type 2 diabetes.

Offerors must review the attached DOH56071 RDQ. Each Offeror must submit a technical proposal and a completed cost template as part of its total Proposal before the opening time on the proposal due date. Further, the Offeror must title the electronic files with “ DOH56071- RDQ #SRC0000027576- Technical Proposal ” and " DOH56071- RDQ #SRC0000027576- Cost Template ” as appropriate.
