Description/Background:

The City of Chino Hills is seeking qualified firms to perform routine preventative maintenance, extraordinary maintenance, and additional maintenance and emergency response of traffic signal systems for locations which are the responsibility of the City of Chino Hills, in accordance with Exhibits A, B, and C. The maintenance shall be extended to include future signal installations at the unit contract price and the Contractor may be required to assist in final inspection.  The Contractor is to furnish all tools, equipment apparatus, facilities, labor services and material, and perform all work necessary to maintain publicly owned traffic signals., flashing beacons and lighted crosswalk facilities.  The Contractor shall provide an electronic photo log and written inventory of all maintenance items.

Special Requirements:
Proposals must be for the entire scope of services outlined in this Request For Proposal (RFP). Incomplete proposals will not be considered. The proposer shall submit a package clearly marked on the outside as “PROPOSAL FOR TRAFFIC SIGNAL MAINTENANCE AND EMERGENCY ON-CALL SERVICES” to the City Clerk’s Office. The package shall contain the following: Three (3) bound copies and one (1) electronic copy (flash drive is preferred method, emails not accepted) of the proposal; One (1) original and one (1) copy of the cost proposal in a separate sealed envelope clearly marked "Sealed Cost Proposal" and a signed W-9.
Miscellaneous:
Proposer must be willing to enter into an agreement with the City of Chino Hills. A copy of the agreement accompanies the full RFP. Any exceptions to the standard agreement must be included with the proposal.
