A licensed contractor will need to inspect the burn building and advise us of necessary repairs in an effort to keep the burn building in service and safe for all students, employees and fire departments.

Dept. of Commerce - State Fire Marshal Office / Ohio Fire Academy – Live Burn Training Building:
To furnish all labor, and equipment to perform refractory tear out and install new block, brick,
window seal.

SCOPE OF SUPPLY:
1. Mobilization and Demobilization
2. Refractory lining installation
3. Project Management
4. Labor / Work specific Tradesmen / Supervision
5. Equipment and Tools

SCOPE OF WORK:
1. Time & Materials, surveying multiple areas, both interior & exterior masonry repairs of live fire burn rooms / communicating
hallways, and stairwell to make all repairs as needed.
2. Repairs needed to any loose or broken block in wall and corners, exterior and interior.
3. Floor stairwell repairs around doors as needed.
4. Repair any new washed-out joints in hallways.
5. Any repairs needed to top of walls as needed.
Labor and Equipment & precast shapes, mortar to perform the above scope of supply and scope of
work with the following schedule.

SCHEDULE:
Total Scope of Work Timeline includes:
1. Working (one) (8) hr. shift Monday - Friday excluding Holidays and Saturday and
Sundays.
2. (4.5) Working Days needed to complete the project.

DELIVERY
1. To be in cooperation with OFA schedule- after receipt of Purchase Order to start work.

TERMS:
1. Proposal and price are valid for 120 days .
Terms Net 30 days billable upon completion
