Madisonville, KY 42431 There is no receiving dock at this facility. The agency does not have equipment to unload the items from the truck. Therefore, it shall be the responsibility of the successful vendor to provide for the unloading of items from the truck. There is a large bay door that opens to a large space for the placement of the equipment. Delivery will coordinated with the site supervisor.
