Amendment 1 is to update language in SOW Paragraph 2 regarding licensing agreements to allow initial and provisional licenses. Amendment 1 also adds language to the solicitation posting description to clarify submission instructions already incorporated in solicitation by reference to FAR 52.212-1.
Vendor will provide on-site childcare during full event hours from 0900-1600. Staff will manage check-in and check-out procedures. Parents will drop off children starting between 0815 and end at 0850. Caregivers will supervise children while parents attend mandatory briefs thru out event. Throughout the PRE Yellow Ribbon program, there must be appropriate child-to-staff ratios in accordance with West Virginia guidelines, age-appropriate activities, and a structured daily schedule that includes, but is not limited to, crafts, indoor play, reading books, and similar activities.
Offers will be submitted to the Contracting Officer by email no later than the date and time published in the solicitation posting. Offers may be submitted on the SF 1449 or through other contractor proposal document IAW FAR 52.212-1 as long as it shows, at a minimum: The solicitation number; The time specified in the solicitation for receipt of offers; The name, address, and telephone number of the offeror; A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; Terms of any express warranty; Price and any discount terms; "Remit to" address, if different than mailing address; A completed copy of the representations and certifications at Federal Acquisition Regulation (FAR) 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); Acknowledgment of Solicitation Amendments; and If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
