Private Residence - Contractor must be enrolled w/SC Medicaid as BHDD-OIDD waiver service provider-Environmental Modifications. Only bids from qualified enrolled providers accepted. Remove existing tall-walled bathtub and replace with a low-threshold walk-in shower. Install anti-scald shower controls w/ detachable handheld shower head. Install grab bars. Install LVT waterproof flooring in bathroom. Trim, paint, drywall as required. Contact case manager for additional details.
