DHS is seeking to connect people who are eligible for Home and Community Based Culturally Responsive services for the Historically Underserved Communities Throughout the State.
DHS and Home and Community-Based Services are seeking to connect Medicaid eligible persons to provide culturally specific and responsive available services with historically underserved communities throughout Minnesota.
The term of the initial grant period is anticipated to be from July 1, 2025 through June 30, 2026. Extensions/continuation awards will be considered, with the option for a one-time extension.
