Statement of Work

Background

The Nevada Skies Youth Reginal Treatment Center (NS YRTC) is seeking qualified vendor(s) whom can provide commercial plumbing service at Nevada Skies Youth Reginal Treatment Center propane delivery method, Wadsworth, Nevada. Service(s) requested: Installation of a new propane delivery line from current on-hand propane tanks, to building 104 that meets or exceeds industry standards.

Contractor to install, test and monitor new propane delivery line, with all safety devices and systems required by law, local and state mandates, or regulatory means.

Contractor to abandon existing propane line and keep in place.

Contractor is responsible for locating underground utilities (water, power, etc..).to avoid damaging existing utilities.

Clean work area, dispose of properly the previous supplies and materials, parts and ruminants of the old system. Dispose of properly the packing materials, crates, containers or the like for the new system. Ensure the following:

• Function test

• Protection, covers, or caps to prevent contamination

• Cleaning of all work area, ensure safe passage to exits, walkways, or pathways, no less than 36” at doors, driveway, automobile path, road, or passageway

• Replacing any removed, cut or altered draining or excess run-off of water path, scope or pathway to include both earthen, concreate, cement or rock(s)

• Instruction manuals, forms, cards, papers or the like to accompany the device(s)

Copies of all licenses, insurances, safety plans, product labels, Safety Data Sheets, and any necessary training documents will be furnished upon request.

Products or chemicals to be used will be fully compliant and according to federal, state and local regulations.

Service Frequency: Warrantee on labor and parts for no less than one year.

The area of excavation, installation and site original state must be clear, clean, use of said previous materials and

supplies and trained government personnel on use, safeguarding and service maintenance as required. Installed by a certified technician or experienced technician. This includes utilization of required chemicals, certified equipment, and other certified requirements to maintain the device as needed. System Requirements Per vendor’s recommendations. Contract Awardee Responsibilities & Tasks Support and Maintenance Services Vendor shall provide a quality device, test, check and follow-up on installation and service, should end user have any questions or concerns about its use, cleaning, maintenance or records of the above. Minimal disruptions in service of this device is utmost. Training Basic training requirements. Web Requirements None stated. Period of Performance (POP): N/A Minimum Qualifications Contractor shall have a minimum of 05 years of experience as a plumber / installer. Preferred Evaluation Method Best value is the Preferred method of evaluation.
