I. General
You are invited to submit an application for pre-qualification for RFPQ #26033 ATHENA INDUSTRIAL
RECYCLED WATER TRANSMISSION LINE SECTION 2. Pre-qualification applications must be
received at the office of the Unified Government of Athens-Clarke County Purchasing Division/Finance
Department, 375 Satula Avenue, Athens, Georgia 30601, no later than 2:00 P.M. ET, Tuesday, May 5,
2026.
A MANDATORY pre-application conference to respond to all inquiries with regards to this
project will be held at 10:00 A.M. ET, Friday, April 17, 2026 at the Athens-Clarke County Public
Utilities Training Room in the basement of the Water Business Office at 124 E. Hancock Avenue,
Athens, GA 30601 to review an overview of the scope of the work.
A. Pre-qualified vendors will be invited to submit cost proposals on the ensuing ATHENA
INDUSTRIAL RECYCLED WATER TRANSMISSION LINE SECTION 2 A list of names of
vendors providing cost proposals may be obtained from Georgia Procurement Registry or via email
request to accbids@accgov.com after the application due date and time stated herein.
B. The Owner reserves the right once RFPQ and costs proposals have been submitted to not move the
project forward.
C. The work included in this project consists of furnishing all personnel, superintendence, skilled and
unskilled labor, materials, equipment, services, and all work necessary for the construction of a
Recycled Water Transmission Line, including mobilization; erosion and sediment control
measures; open-cut and trenchless installation of 2,400 linear feet of 16-inch PVC water main
piping; fittings, valves, blow-offs, air/vacuum release assemblies, and connections to existing
water mains; hydrostatic pressure testing, leakage testing, and disinfection in accordance with
AWWA standards; surface restoration including roadway, sidewalk, and driveway replacement;
permanent seeding; utility coordination; and all incidental work required for a complete
installation.

interpretations and any supplemental instructions will be in the form of written Addenda to the RFPQ
which, if issued, will be posted prior to the Application submittal deadline. All Addenda shall become
part of the RFPQ.

INDUSTRIAL RECYCLED WATER TRANSMISSION LINE SECTION 2 is required to
complete and submit the Application for Pre-Qualification documents (including all instructions
and forms) regardless of whether the vendor has previously pre-qualified for other work for the
Unified Government of Athens-Clarke County.
Cost proposals received by the Unified Government of Athens-Clarke County for this Project from
vendors who have not been pre-qualified through this process will be rejected.

G. Documentation provided must be specific and the data given must be clear and comprehensive.
Documentation provided must pertain specifically to the entity that will bid for and install the project,
not an affiliated company, parent firm or entity.
II. Pre-Qualification Determination
NOTE: All criteria will be based on Pass/Fail scoring.
A. Applicants may not be deemed pre-qualified if:
1. The Applicant fails to submit a responsive Application, including failing to provide all
required documentation;
2. The Applicant fails to meet all the Technical Experience Requirements;
3. The Applicant fails to meet all the Corporate Experience Requirements;
4. The Applicant, or any of its affiliated companies, is a party to more than one Application;
5. Reasonable grounds exist that Applicant is involved in collusion among other applicants; or
6. The Applicant fails to execute the Disclosure Affidavit.
B. Final determination of Applicant's pre-qualification status for this project rests solely with the Unified
Government of Athens-Clarke County.
III. Qualification Criteria:
A. Background Information:
1. The project includes construction of a 16-inch Raw Water Transmission Line serving the

Athena Industrial area.
2. Installation methods include both open-cut trenching and trenchless construction.
3. Construction will occur within active roadways and developed areas, requiring traffic control
and coordination with utility owners.
4. Work includes testing, disinfection, and full surface restoration following construction.
B. Technical Experience
1. Contractors shall demonstrate experience installing large-diameter (16-inch or greater) water
transmission mains.
2. Contractors shall demonstrate experience with trenchless installation methods, including free-
bore or similar techniques.
3. Contractors shall demonstrate experience performing work within active roadways, including
traffic control and surface restoration.
4. Contractors shall demonstrate experience with hydrostatic testing, leakage testing, and
disinfection of water mains in accordance with AWWA standards.
5. Contractors shall demonstrate experience coordinating with utility owners and regulatory
agencies.
C. Corporate Experience:
1. The Applicant has operated under the current corporate name for the last five years.
2. The Applicant currently has sufficient resources to provide insurance requirements for the
project.
3. The Applicant has not defaulted on a project or failed to complete a project within the last ten
years.
4. The Applicant has not filed for bankruptcy nor been judged bankrupt at any time over the last
ten years.
5. The Applicant has a history of completing projects consistently on time and within the original
project budget/bid amount.
6. The Applicant has never abandoned a project (even temporarily) during a dispute.
7. The Applicant does not have a history of being involved in litigation or filing claims against
Owners, Construction Contract Administration Firms or Design Firms.
8. The Applicant is not, nor any of its affiliated companies, currently involved in a dispute,
formal claim, or litigation with the Owner, or with any authority or organization with which
the Owner has a vested interest.
9. The Applicant, or Applicant's listed installation subcontractor(s), is a Licensed Utility
Contractor in the State of Georgia.
IV. Required Documentation
A. Application Cover & Disclosure Affidavit - to be completed by all applicants
Applicant must complete and submit the Contractor Pre-Qualification Application Cover and
Disclosure Affidavit (attached).
B. Technical Experience
Applicants shall submit documentation demonstrating compliance with the Technical Experience
requirements outlined in Section III.B. For each project submitted, the Applicant shall provide
complete and accurate information using the Technical Experience Project Information Form
provided herein or an equivalent format. Submitted documentation shall clearly demonstrate
experience with large-diameter water transmission mains, trenchless installation methods, work

within active roadways, utility coordination, and testing and disinfection of water mains in accordance
with AWWA standards.
C. Corporate Experience
1. Provide name, address and telephone number of Applicant's corporate headquarters, regional
offices, subsidiaries and affiliates, if any. Provide name, title, and biographical summary of
pertinent corporate officers. Provide any corporate names the applicant has operated under
during the last five years and the reason for the name change. Provide the state and date of
incorporation.
2. Provide a statement of bonding capacity, name of surety company, with agent contact persons,
and telephone numbers. Define current maximum approved bonding program, allowable
single project limit and current bonded work backlog.
3. Provide a statement that Applicant has not defaulted on a project nor failed to complete a
project within the past ten years. If this is not the case, explain.
4. Provide a statement that Applicant has not filed for bankruptcy or been judged bankrupt at
any time over the past ten years. If this is not the case, explain.
5. Provide a statement that Applicant has not been involved in liquidated damages in the past
five years. If this is not the case, explain.
6. Provide a statement that the Applicant has never abandoned a project, even temporarily,
during a dispute. If this is not the case, please explain.
7. Provide a statement whether Applicant has or has not been involved in litigation as a plaintiff
against an Owner, Design Firm or Construction Contract Administration Firm, or served the
Owner with a claim for additional compensation prepared by an attorney or a claims
consultant, excluding routine change order requests, in the past five years. If Applicant has,
explain. List any lawsuits or administrative actions to which the Applicant is currently a party
or has been a party (either as a plaintiff or defendant) during the past ten years. For each suit,
list all parties and indicate whether any party was a bonding company, insurance company,
an Owner, or other. Identify the project giving rise to the suit or administrative action; explain
the basis of the claim, and whether a settlement was reached or a judgment entered into for or
against the Applicant, or the Applicant's bonding company or Insurance Company.
8. Provide a statement that the Applicant, as well as all of its affiliated companies, is not involved
in any dispute, formal claim, or litigation with the Owner, nor any authority or organization
with which the Owner has a vested interest. If this is not the case, please explain.
9. Include a copy or other evidence of the Applicant's, or listed installation subcontractor(s),
Georgia Utility Contractor License.
V. Supplemental Requirements
List all Supplemental requirements needed by vendor.
1. Applicants shall provide a list of all similar water transmission line projects completed within
the past five (5) years, including project size, pipe diameter, installation method, month/year
completed, and owner contact information.
2. Applicants shall provide a list of all projects currently under contract, including contract value
and anticipated completion dates.
3. Applications shall be organized in the order presented in this RFPQ, clearly indexed, and
include all required forms and documentation.
4. Non-pertinent marketing or promotional materials are discouraged.
5. Application Format: The Application must include responses to all information requested.
The Application shall address required documentation in the order identified above, indexed
and clearly identified. Additional data may be submitted as deemed appropriate to support or

augment the required documentation. Submittal shall be prepared on standard 8-1/2" x 11"
letter size paper, with text on one side, using 12-point font, and bound on the left side. No
page limitation has been established for the application submission, but non-pertinent
"boilerplate" materials are discouraged.
VI. Supplemental Information
1. The Owner will not be liable in any way for any costs incurred by any Applicant in the
preparation of its Pre-Qualification Application nor participation in any discussions.
2. The Owner reserves the right to accept or reject any or all Pre-Qualification applications.
The Owner, upon receipt of the Application, shall have the right to investigate work
performed by the Applicant on projects referenced by the Applicant, or any other projects
performed by the Applicant. Investigations will be performed by Owner's personnel or
their representatives as deemed appropriate by the Owner, which may include site visits
and interviews with project Owners and Construction Contract Administration staff. The
Owner reserves the right to interpret the criteria, to waive any criteria, and to determine
whether applicant meets the intent of the criteria, where such action is in the best interest of
the Owner as determined by the Owner.
3. An Application submitted by a Joint Venture will not be allowed by the Owner.
4. If the Applicant does not meet all the corporate and technical experience criteria, additional
consideration may be given as follows:
a. If the corporate name of the Applicant is a result of an acquisition or a merger, the
Applicant may use the corporate and technical experience of the parent corporation,
a sister corporation or a subsidiary corporation, if satisfactory evidence can be
provided to demonstrate that essentially the same core management team and staff
which created the experience is directly involved in the day-to-day operations of the
Applicant.
b. If the corporate name of the Applicant is a result of the formation of a subsidiary by
a parent company in order to compete in a different market, e.g. union versus non-
union territories, the Applicant may use the corporate and technical experience of the
parent corporation, a sister corporation or a subsidiary corporation, if satisfactory
evidence can be provided to demonstrate that essentially the same core management
team and staff which created the experience is directly involved in the day-to-day
operations of the Applicant.
5. Should any Applicant not be deemed pre-qualified by the Owner, the Applicant may respond
within three business days of being notified of pre-qualification status to the Unified
Government of Athens-Clarke County Administrator requesting reasons for not being deemed
pre-qualified. The Applicant may then provide further written information in an attempt to
correct such deficiencies within three business days of receiving reasons for not being deemed
pre-qualified. This will be an informal appeals process.
6. For the purpose of this Application and its review, "project" shall be defined as a single
project, including all associated change orders.
7. The Project will require the Contractor to have at least one Superintendent, which meets the
Pre-Qualification Technical Experience Criteria, in responsible charge of the Project during
installation activities.

8. Due to potential time length of this project, the Owner reserves the right to approve any
changes in personnel prior to assignment to this project.
9. The Owner anticipates advising Applicants of their Application approval/rejection by late
May 2026.
VII. Insurance and Bonds Required for Contractors
Contractor shall procure and maintain insurance which shall protect the Contractor and ACCGov from any
claims for bodily injury, property damage, or personal injury which may arise out of operations under the
agreement. Contractor shall procure the insurance policies at the Contractor's own expense and shall furnish
ACCGov an insurance certificate listing ACCGov as certificate holder. The insurance certificate must
document that the liability insurance coverage purchased by the Contractor includes contractual liability
coverage to protect ACCGov. The insurance certificate must provide the following information:
1) Name and address of authorized agent
2) Name and address of insured
3) Name of insurance company (licensed to operate in Georgia)
4) Description of coverage in standard terminology
5) Policy period
6) Limits of liability
7) Name and address of certificate holder (ACCGov)
8) Acknowledgement of notice of cancellation to ACCGov
9) Signature of authorized agent
10) Telephone number of authorized agent
11) Details of policy exclusions in comments section of insurance certificate
12) Additional Insured Endorsement
Note: It is the intent of ACCGov that certificates of insurance should comply with the pertinent Rules of the
Commissioner of Insurance, which are found in Chapter 120-2-103 of the Official Compilation of the Rules and
Regulations of the State of Georgia.
Each policy shall contain a provision that coverage afforded under the policies will not be canceled (or not
renewed or allowed to lapse for any reason) until at least thirty (30) days after Owner has received notice
thereof as evidenced by return receipt of registered letter. All policies must be issued by an insurance company
licensed to do business in the State of Georgia, with a minimum AM Best rating of A-, and signed by an
authorized agent.
The Contractor must submit, along with insurance certificate(s), an additional insured endorsement for all
coverages as allowed by statute naming the Unified Government of Athens Clarke County, and a notice of
cancellation endorsement.
Coverages and Limits Required:
Workers Compensation Insurance (per Occurrence) in the amounts of the statutory limits established by the
General Assembly of the State of Georgia (a self-insurer must submit a certificate from the Georgia Board of
Works Compensation stating that the supplier qualifies to pay its own workers compensation claims). In
addition, the supplier shall require all subcontractors occupying the premises or performing work under the
contract to obtain an insurance certificate showing proof of Workers Compensation Coverage with the
following minimum coverage:

Bodily injury by accident - per employee $1,000,000
Bodily injury by disease - per employee $1,000,000
Bodily injury by disease - policy limit $1,000,000
Commercial General Liability (CGL):

Additional Insured: The vendor or contractor shall add "the Unified Government of Athens-Clarke County, its
officers, employees, and agents" as an additional insured under the commercial general, automobile, builder's
risk, contractor's pollution liability, and umbrella liability policies.
Contractors are responsible for making sure all sub-contractors have the required insurance coverage.
Performance Bond and Payment Bonds:
a. Any bid exceeding $100,000 requires a performance and payment bond in the amount
of 100% of the job. No material deviation from this language will be accepted. These
documents must be filled out completely and notarized with appropriate power of
attorney form attached.
b. The bonding company will only be accepted if it is in good standing within the state
of Georgia through the Insurance Commissioner's office. It must also be rated A or
better with AM Best Company. The bonding company must appear on the Federal
register of approved companies.
