CHA Modernization

Project Description: Davis County (“the County”) on behalf of the Davis County Health Department (“Health Department”) is seeking a qualified service provider (“Vendor”) to provide a web-based platform that compiles and integrates available population data from national, state, and local sources. This platform will support the development and presentation of the Community Health Assessment (“CHA”), including the integration of publicly available and locally collected quantitative and qualitative data, visual storytelling, and printable reporting. This platform may also include a data dashboard component, where all compiled data can be accessed, explored, and filtered to support real-time decision-making. The selected Vendor will be responsible for delivering a secure, user-friendly, and customizable platform, along with training and support. It will serve as the foundation for all community health improvement efforts and performance tracking. The project is anticipated to begin January 2026 with the public CHA release happening by January 2028.
