Forklift and Pallet Lift Preventative Maintenance and Repairs

Opportunities for Ohioans with Disabilities (OOD) is requesting quotes for a preventative maintenance service twice a year, as well as any additional repairs (outside of preventative maintenance) as requested, on  one (1) Toyota brand forklift truck and three (3) Toyota brand pallet lifts. The agreement will have an estimated start date of July 1, 2025 to June 30, 2027.

Please attach a formal quote which includes contract nubmer and product codes, if applicable.

A) OOD shall base the award on lowest cost.

B) OOD will also base the award on the respondent that is responsive, responsible, and able to complete the work described in the summary.

C) A formal quote must be attached.

D) Respondent shall complete and submit the Certification Statements form if seeking a Buy America, Buy Ohio, and/or a Veterans Preference. If not seeking a preference, Respondent does not need to return the attached form.

Mailed, emailed, or faxed responses will NOT be accepted.
