DDS West Region Two 4 Bed CLAs

The State of Connecticut, Department of Developmental Services (DDS) West Region is seeking proposals to develop two four (4) bed Community Living Arrangement (CLA) that will provide supervision, overall operations and administrative duties. The CLA will assist Individuals with the acquisition, retention, and improvement in skills related to activities of daily living. The CLA will support individuals in developing community relationships and participation, family relationships and actualizing IP goals. The CLA program will be required to support nursing, clinical, behavioral and psychiatric needs of the individuals, as well as medical and dental providers. These services will need to be clearly defined in the proposal. Clinical support will need to be provided as needed, such as physical therapy, occupational therapy and speech therapy.
